Care Coordinator - Red Deer, Alberta

Temporary position: 6 month contract with full time hours. Monday to Friday

Reporting to the Director of Operations or designate, the Care Coordinator is primarily responsible for scheduling, coordinating and providing direction for field employees in the delivery of after hours services.

The Coordinator works within and promotes a positive team environment and communicates effectively with all members of the team.


Your role

  • Ensure that the field employee understands their role, care to be provided and is competent in providing required care.
  • Use the scheduling software provided to its fullest capacity to create efficiencies in all aspects of the coordinators role as per specified standards
  • Manage requests for service and coordinate assignment of care
  • Manage changes to service provision
  • Maintain knowledge of field employee's work preferences, availability and competencies
  • Maintain electronic client and field employee's files where required
  • Document professionally, appropriately, and in a timely manner Interact with clients to:
  • Identify their needs and preferences
  • Inform them of whom the field employee will be
  • Inform them of day/time service will be provided
  • Ensure that they are satisfied with the quality of care being provided
  • Resolve concerns Document client concerns for follow up with referring agencies
  • Knows and adheres to the formal and informal departmental goals, standards and policies and procedures
  • Work collaboratively with Field Supervisors as necessary and document for follow up as required
  • Participate in office meetings and additional training as required
  • Work with an awareness of health and safety and cooperate towards improved health and safety conditions at work.

What you will need to be successful

  • Proficiency in Microsoft Office Suite, and the ability to learn new programs specific to the business
  • Demonstrate excellent telephone etiquette
  • Scheduling experience preferably in the health care field is considered an asset
  • Be able to empathize with the complexity and difficulty of each client or field staff situation
  • Be a detailoriented, adaptable and customer focused team player
  • Must have the ability to work in the office this is not a remote position
  • Effective communication skills both verbally and in writing
