Job summary
We are seeking an experienced registered nurse (RN), physiotherapist, occupational therapist, or social worker (RSW) to join our team as a Care Coordinator. As a Care Coordinator, you will assess patient care needs and eligibility, provide access and referrals to community services, and engage with patients.
