Primacare Living Solutions is currently looking to fill a role of a social worker for our Long-Term Care Home in St.
Catharine's.
In this role the incumbent will provide social work services 4 days a week to our residents and their families.
Responsibilities:
- Providing Support and Counselling to Residents and Families
- Facilitating admissions, discharges and transfers
- Facilitating tours of the home for new residents and families
- Attending and participating in Care Conferences
- Work with all members of the interdisciplinary team
Qualifications
- Degree in social work
- Membership in the Ontario College of Social Workers
- Experience working in a similar or same role in Long Term Care
- Knowledge of applicable regulations

Salary:
$45,000.00 per year
Schedule:
- 8 hour shift
Experience:
Long Term Care: 2 years (preferred)
Licence/Certification:
- Ontario College of Social Workers (preferred)
