  • Our staff support between 410 residents per shift with varying degrees of contacts depending on daily needs.

The main purpose of this position


As the House Support Coordinator you create and maintain a safe, clean and welcoming environment for the people that we provide support to within our residences.

You work onsite within our residences and monitor health and safety; provide supportive counselling; implement house rules, policies and procedures; and respond to crisis situations.

You provide a supportive, safe, and welcoming environment for those within the residence.


The House Support Coordinator will support mental health and substance use needs with a special focus on supporting residents experiencing social and physical harms associated with chronic substance use through providing support and resources for harm reduction and prevention.

Our work is to maintain housing stability and tenancy with harm reduction approaches in order to improve people's wellness.


Responsibilities

  • Health and wellness support_
  • Oversee the operations of the residences while on shift.
  • Support residents with mental health concerns or crises utilizing supportive counselling, strengthbased approach, crisis intervention skills and engaging appropriate resources as needed.
  • Engage residents who use drugs into respectful conversations about safer drug use, safer sex, overdose prevention and reduction of other harms associated with drug use.
  • Provide harm reduction and overdose prevention resources
  • Monitor drug use (including injections, and inhalation) and respond to overdose with injection naloxone and engagement of emergency and/or crisis services when required.
  • Perform resident bed checks.
  • Interact with residents consistent with individual needs and plans.
  • Provide assistance with congregate living routines, including socialization and activities of daily living (I.e., cleaning, cooking, grocery shopping).
  • Will assist residents with household and individual room upkeep during shift.
  • Ensure health and safety of home, including ensuring maintenance and cleanliness of shared spaces.
  • Provide symptom management and monitoring as related to mental and physical health status and engage additional resources (including clinical supports) as required.
  • Provide support associated with medications and treatment plans.

  • Maintain accurate case record logs and reports
  • Additional documentation can include Incident Reports and Health and Safety reporting.
  • Regularly consult with the Residential Program Supervisor, Grace House to provide feedback and housing updates

Knowledge and skills necessary to be successful in this role

  • Relevant postsecondary education in the social services field coupled with relevant experience in Mental Health and substance use
  • Registered with the Ontario College of Social Workers and Social Service Workers is an asset
  • Proven ability to connect, empathize and work alongside people experiencing Mental Health and substance use concerns
  • Experience in harm reduction best practices and overdose prevention
  • Proven ability to support someone where they are at
  • Crisis intervention and deescalation skills and relevant trainings
  • Exceptional interpersonal skills that promote strong relationships, positive team building, conflict resolution and solution focused problem solving
  • Knowledge of recoverybased program philosophy. Proven ability to work from recoverybased and holistic wellness philosophy
  • Strong working knowledge of relevant community resources
  • Experience using the following tools is considered an asset: OCAN, LOCUS, GAINS
  • Excellent written and documentation skills, with the ability to time manage a mix of administration and direct service
  • Demonstrated sound judgment, integrity, patience
  • ASIST Trainings, NVCI Trainings, etc.
  • Proven ability to take initiative and work independently, as well as in a team based environment
  • Proven technology proficiency, including expertise with Microsoft Products and ability to work on Electronic Healthcare Record
  • Selfawareness, selfreflection, and commitment to ongoing learning and personal development
  • A valid driver's license and access to a vehicle
  • Current First Aid & CPR certification and Naloxone Training

  • Works 40 hours per week with a mix of weekdays, evenings and weekend shifts
