We are seeking a dynamic, highly motivated
Client Care Worker who demonstrates our values of compassion, accountability, responsiveness, and empowerment; to our clients, families, and each other.
The
Client Care Worker will be working in the Supportive Housing Program to support 10 adults living with a dual diagnosis.
This position is a
unionized, permanent, full-time position. The rate of pay is
$ 20.54 per hour with a $0.50/hour premium for work after 11 pm.
Location:
Hamilton.
**Key Responsibilities: - Provide specialized and individual services to individuals with complex special needs, behavioural and mental health challenges in a residential setting.
- Offer crisis intervention as appropriate; using safety plans, care plans, emergency intervention, and oncall as appropriate.
- Assist clients with supports as needed in regards to activities of daily living, life skills, and recreation.
- Regular checks on clients to ensure they remain safe and stable.
- Cleaning of the program common areas, and any preparation that can be completed to assist in the following shifts.
- Documentation such as case notes, med documentation, and cash counts.
- Client medication administration and basic health monitoring.
- Team meeting attendance, communication, and shift change documentation.
- To interact with clients in a dignified and professional manner acknowledging their strengths and following a clientcentered approach.
What you will bring:
- Postsecondary degree/diploma in Social Services, or relevant equivalent required.
- Current First Aid/CPR and NVCI certifications.
- Concurrent Disorders, Mental Health, and Disability Management or equivalent certificate is an asset.
- Minimum of 6 months to 1 year of experience, effectively working with marginalized populations and a demonstrated ability to form rapport with target populations preferably in a residential setting.
- Experience providing clientcentered services within a multidisciplinary team of providers.
