Description

The Caseworker provides a high impact case management approach that focuses on identifying and addressing root causes for families living in poverty.

Utilizing a strengths-based approach to interview, assess, and formulate a plan of action to assist families in moving from crisis to stability.

This position provides casework services to families through the Pathway of Hope (PoH) approach.


KEY RESPONSIBILITIES:


Direct Case Management Services

  • Engage and build rapport with the target population.
  • Provide client intake and ongoing assessments to Pathway of Hope participants.
  • Perform all aspects of the case management process using the Pathway of Hope framework (prescreening, intake, assessment, goal planning, action, transition, and followup)
  • Work with Pathway of Hope participants and ministry unit teams to coordinate ongoing goal setting and action plans; schedule regular meetings to review progress and engage in further planning.
  • Effectively maintain records and detailed case notes for every interaction
  • Assist participants in making connections and accessing community resources, supporting participants as appropriate.

Ministry Unit Coordination

  • Assist with the Ministry Unit to form and maintain a local Pathway of Hope team including Officers, employees, congregation members, volunteers, and community partners, to work with participants from a holistic perspective.
  • Facilitate ministry unit access to the services of the THQ Pathway of Hope team including case consultation and technical assistance, training, and development, as well as evaluation and reporting.
  • Ensure program policies and procedures are followed to maintain integrity, accountability, and program outcome measurements.

Communication and Reporting

  • Attend training sessions that support the initial implementation phase and ongoing training for the sustained delivery of Pathway of Hope, as well as ongoing training and meetings as required.

Community Networking and Partnerships

  • Network with other service providers and maintain relationships with community agencies to provide comprehensive services for participants.

Community & Family Services Responsibilities

  • Assist with client food bank appointments and activities.
  • Assist with other Community & Family Services programs as required, i.e., Christmas food and toy hampers, budget management, food bank, etc., ensuring all clients are treated with dignity and respect.

Health & Safety

  • Ensure that the work area is maintained regarding appearance, condition and safety as recommended by Public Health etc.
  • Required to wear appropriate PPE which may include masks, gloves, face shields, etc., as per Public Health guidelines/recommendations.
  • Ensure the safety and comfort of the ministry unit by maintaining cleanliness and food safety etc.
  • Work in compliance with the OH&S Act and Regulations and abide by The Salvation Army's health and safety policies and procedures.
Perform other duties as required.


WORKING CONDITIONS:


  • This is a permanent fulltime position based on 37.5 hours per week.
  • Some flexibility in scheduling required, some weekend and evening work may be required.
  • The Supervisor will set the hours of work and work schedule.
  • Working environment is typically in the office in generally agreeable conditions.
  • May deal with difficult people and verbal abuse.
  • Ability to lift/move up to 50 lbs.
  • This job requires reaching, bending, and stooping frequently, working in a sitting, and standing position for periods of time.
  • Some travel locally and throughout the Grey Bruce area will be required, with possible travel to meetings outside the immediate area for meetings (i.e. DHQ, THQ).

QUALIFICATIONS AND EDUCATION REQUIREMENTS:


  • Completion of a two (2) year Community College Diploma in a human services field, i.e., Social Services Worker.
  • A University Degree in a relevant field of study would be a definite asset, i.e., Bachelor of Social Work, or a Bachelor of Arts in Social Services).
  • Alternative combinations of education and experience may be considered.

EXPERIENCE AND KNOWLEDGE:


  • A minimum of two (2) years' related experience, preferably in community engagement or a similar community outreach role
  • Hold a valid Ontario Class "G" drivers licence, and provide a copy of a driver's abtract for review and verification that is satisfactory to The Salvation Army, in its sole discrection. A program vehicle will be used for travel when required

SKILLS AND CAPABILITIES:


  • Training in deescalation and intervention strategies is an asset.
  • Training (or willingness to be trained) in Naloxone Overdose kit will be required.
  • Demonstrated ability to connect with and establish relationship with those who are marginalized.
