Position Title: Occupational Therapist

Location: Calgary, AB

Term: Full-Time, Contract (March – August 2026)

Reports To: Provincial Manager, Healthcare Operations

Salary: $ $53.80/ hour

I. Scope and Summary of Primary Responsibilities

The Occupational Therapist (OT) plays a key role in delivering and evaluating Clear Path Home, an Occupational Therapy–led vision rehabilitation and clinical service coordination model designed by Vision Loss Rehabilitation Canada to support adults with vision loss during transition from hospital to home or community living.

The incumbent provides in-person occupational therapy services and collaborates virtually to support safe discharge planning and transition to home. This role contributes to testing and validating the model's feasibility, effectiveness, and scalability in Calgary (with virtual and periodic work in Edmonton), supporting improved functional outcomes and safe community reintegration for adults with vision loss.

Role Accountabilities

Client Outcomes

  • Improved safety and functional independence (measured by COPM and/or functional assessment tools)
  • High client satisfaction with rehabilitation and transition supports
  • Increased proportion of clients safely transitioning to their residence or appropriate alternate housing

Program & Evaluation Outcomes

  • Demonstrated feasibility of the OT-led model in Calgary and Edmonton through documentation of operational, staffing, and workflow requirements
  • Evidence to inform scalability of virtual clinical service coordination
  • Where data access permits, contribution to indicators related to reduced readmissions and improved patient flow

II. Essential Functions

This position entails, but is not limited to the following:

Clinical Assessment & Intervention

  • Conducts comprehensive occupational therapy assessments across diverse settings, including hospitals, seniors' residences, long-term care, and client homes.
  • Develops individualized, measurable, and time-limited treatment goals and plans in alignment with Project parameters and initiates therapy with informed consent in accordance with the Health Care Consent Act.
  • Provides direct intervention, consultation, and education to clients, families, caregivers, and health care professionals.
  • Deliver instruction in adaptive and independent living skills, including personal care, household management, communication, technology use, and foundational indoor mobility strategies.
  • Assesses and recommends adaptive aids, assistive devices, and equipment to support functional goals.
  • Provides instruction in spatial and sensory awareness, pre-cane techniques, mobility aids, and environmental assessment.

Interdisciplinary Collaboration and Service Coordination

  • Participates in case conferences and collaborates with interdisciplinary teams to support holistic care.
  • Support coordinated transition planning without creating additional referral pathways or burden on hospital teams.
  • Performs ongoing reassessment, documents progress and communicates outcomes to ensure continuity of care and appropriate referrals.

Documentation & Outcome Measurement

  • Maintains accurate clinical documentation in accordance with Alberta College of Occupational Therapists standards and VLRC policies.
  • Collects and submit required outcome measures (e.g., COPM, client satisfaction)
  • Contributes de identified and/or aggregated data required for Project evaluation and reporting.
  • Identify operational barriers and recommend workflow improvements.
  • Provide feedback to support refinement and scalability of the model and support documentation of processes to inform potential expansion.

Relationships

  • Interacts with clients, their family and caregivers to develop and identify vision related goals and strategies
  • Build collaborative working relationships with healthcare providers, community partners, and allied agencies.
  • Participates in multi-disciplinary team meetings
  • Advocates on client's behalf in various settings, when possible
  • Instructs, delegates to, and supervises Vision Rehabilitation Assistants (where applicable)

Problem Solving

  • Clinical reasoning to address complex discharge barriers and functional safety concerns.
  • Adapt services and interventions for individuals with health conditions
  • Demonstrate cultural sensitivity and awareness of diverse client needs.

Decision Autonomy

  • Exercise independent clinical judgment within professional scope of practice
  • Determine assessment methods, service intensity, frequency, and duration within Project parameters.

Leadership

  • Serve as a subject matter resource on vision rehabilitation within discharge and transition contexts.

Demonstrate initiative in developing client-centered, evidence-informed interventions.

  • Contribute to knowledge-sharing and professional learning related to the Project.

Requirements

III. Qualifications

Knowledge and Requirements:

  • Demonstrated experience in adult rehabilitation, community‑based OT, or complex discharge support, supporting clients with cognitive decline, stroke, brain injury, or complex psychosocial needs.
  • Experience working with clients with vision loss (preferred).
  • Familiarity with Alberta healthcare systems and hospital discharge processes is an asset.
  • Experience working within pilot projects, time‑limited initiatives, or evaluation‑focused programs.
  • Comfort with virtual service delivery and technology‑enabled care.
  • Strong clinical documentation, organization, and communication skills.

Experience and Education:

  • Master of Occupational Therapy Degree
  • Current registration in good standing with the Alberta College of Occupational Therapists.
  • Minimum three (3) years of professional OT experience in relevant settings.

Work Environment:

  • Willingness to work flexible hours
  • Willingness to work in a variety of settings (hospitals, long term care facilities, homes)
  • Regular local travel within Calgary and periodic travel to Edmonton required.
