Summary:


The Brain Injury Association serving the four counties of Peterborough, Kawartha Lakes, Haliburton and Northumberland is a unique supportive organization structured as an Association including individuals with the lived experiences of acquired brain injuries, their care partners and both paid and unpaid staff and a Board of Directors.

Client Service Coordinator (CSC) is a position that has been developed with the intent to ensure that individuals with the lived experience of both non-traumatic and traumatic acquired brain injuries are assisted to reach goals and activities that they hold to be meaningful and life-enhancing.


The CSC is responsible for the support of individuals living with the effects of acquired brain injuries through the provision of person-centered service in the communities where they live.

The CSC carries out the duties within program teams of the Association.

This includes responsibility for providing individualized support and intervention to individuals with ABI in the community through assessment, ongoing individual service planning and evaluation, consultation with other service providers outside of the Association and crisis intervention/referrals.

Job Duties

  • Respect and protect the rights of clients, including but not limited to their right to independence, autonomy, and selfdetermination; their right to choose and practice individual values, beliefs, religion, and culture; their right to privacy and dignity; and their right to live free from retaliation, discrimination, and abuse
  • Providing individualized support and intervention to individuals with ABI in the community through assessment, ongoing individual service planning and evaluation, consultation with other service providers outside of the Association and crisis intervention/referrals.
  • Work with the CSC Supervisor and/or team lead to oversee and assist with the delivery and implementation of program

Responsibilities:


  • Works with individuals to define professional role, boundaries and development of a strong therapeutic relationship with the client
  • Good judgement, the ability to work independently as well as cooperatively with others/team
  • Sensitive to challenges experienced by marginalized groups
  • Identify individual needs and assist in the development of personal plans
  • Demonstrate clientcentred approach
  • Provides support, coaching, and skill development; fosters individual independence as much as possible
  • May involve assisting clients with activities of daily living cooking, cleaning, shopping, banking, garbage, recycling, laundry, other inhome supports, & social/recreational activities. This support could also involve assisting clients to prepare home during infestations by using proper protocols and PPE
  • Prompt clients in regards to personal care, and medication administration, as required; however, we do not provide personal care services
  • Encourage individuals to develop and utilize a support network ensuring that these supports begin with informal and familiar people as well as how to access professional services as required
  • Advocate and assist at the person's request for needed services and supports from other service providers such as social services, including income supports (OW, ODSP, CCPD, LTD, WSIB, auto insurance benefits, etc.), psychiatric, medical, vocational, educational, & housing
  • Provision of crisis resources and development of crisis support plans with other agents
  • Track, review, and evaluate programs that may assist the individuals to achieve their personal plans
  • Ensure that all staff of the Association team is aware of potential issues and crisis as well as the ways to reinforce and encourage individuals to be successful by celebrating achievements, regardless of size
  • Clear, concise and timely client documentation, prepare reports, maintain comprehensive statistics as required on a weekly, monthly and quarterly basis. Client notes are to be updated within 2448 hrs unless approved otherwise
  • Attend weekly staff meetings, virtually or in person as required
  • Attend CSC/CSW team meetings
  • Advocate and raise awareness of brain injury with community partners (working groups, committees, etc.) where appropriate
  • Reports weekly to CSC Supervisor or team lead on support of assigned clients
  • Will discuss issues related to best practices of the role and responsibilities with the team members, CSC Supervisor, and the Executive Director
