Wyndham House programs include two Permanent Supportive Housing Programs, an Emergency Shelter, a Housing Stability Outreach Team, an Integrated Youth Service Hub and a Care and Treatment Day Program.

Wyndham House is a fast-paced and growing organization that works to prevent, reduce and end youth homelessness, as part of the Built for Zero change effort, which is working to end chronic homelessness in Canada.

Wyndham House provides high-quality case management, housing and clinical supports to youth with a variety of needs and complexities.


Job Overview:

The Concurrent Clinician will provide concurrent-capable mental health intervention for youth with a complex intersection of needs.

The concurrent clinician will work with a group of case managers as part of the housing stability team to provide intensive clinical support to clients presenting with complex mental health and substance related needs.

The Concurrent Clinician will work from an evidence-based perspective, have a client-centred approach and remain flexible and adaptable in their role.


Job Responsibilities and Duties:


  • Oneonone clinical intervention with clients
  • Support of the creation and implementation of individual treatment plans
  • Ongoing assessment of new individuals and progress of individuals using evidencebased modalities including cognitive behavioural therapy, mindfulness, dialectical behaviour therapy, interpersonal therapy and traumainformed care
  • Working from a philosophy consistent with a harm reduction approach embedded within the transtheoretical model of change
  • Collaborate with other stakeholders to ensure clarity of care plan for individuals
  • Case consultation with case manager team and programs
  • Completion of ongoing documentation
  • Support case management team in any capacity, including: Liaising with patient's family, referral sources, community providers, etc.
  • Knowledge of community resources or experience building connections
  • Creation of comprehensive mental health assessments
  • Crisis counselling/response

Job Skills and Qualifications:


  • A Masters of Social Work (MSW) or related degree and registration with a relevant college is required
  • 2 Years experience working with vulnerable population in a community setting (prefer experience working with atrisk youth)
  • Working knowledge of the Mental Health Act, Youth Criminal Justice Act, Landlord-Tenant Act, Ontario Works, Ontario Disability Support Plan, and the Child and Family Service Act as they pertain to work with highrisk youth and homelessness
  • Experience working with individuals experiencing an intersection of complex challenges including homelessness, mental health struggles, substance abuse, neurodiversity and complex life circumstances
  • Teamoriented and collaborative approach
  • Strong interpersonal skills in establishing working relationships with staff and community partners to create collaborative support plans
  • Ability to advocate for clients when necessary
  • Ability to assess risk of client safety and develop safety plans with clients
  • Demonstrated crisis management, conflict resolution, problem solving and prioritysetting abilities
  • Clinical training in modalities that are relevant to the position including but not limited to training Dialectical Behaviour Therapy and Motivational Interviewing
  • Experience conducting therapy in a clinical setting is considered an asset

Requirements:


  • Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) in Good Standing
  • Emergency First Aid/CPR
  • Vulnerable Sector Criminal Reference Check
