Jack & Jill Health is a leading telemedicine provider in Canada. We are expanding our offering to include a unique approach to weight management.

We are looking for a Bariatric Educator - Registered Dietitian who can work for us on a contract basis

The Bariatric Educator

Registered Dietitian is responsible for navigating care, providing education and developing nutritional plans for our patients.

Roles and Responsibilities:

Create and establish medical protocols and procedures for triaging patients suitable for the program

creation and distribution of medical information, pamphlets and fact sheets for patients.

Conduct patient intake and establish relationships with patients

Care navigation through our various modalities, treatments and specialists

Educate and advise patients on weight loss and self-management skills

Develop care plans for patients with diabetes (Type 1, 2, GDM, Pre-Diabetes) and metabolic syndrome

Collaborate with the multidisciplinary team (nurse practitioners, medical director, pharmacist)

Teach administration of injectable medication effectively, and self-monitoring of blood glucose and blood pressure as per medical directives

Training of additional staff and any new educators, as required

Communicate with primary care providers

Provide excellent patient care and customer service

Other duties as required by management

EDUCATION/EXPERIENCE
A valid RD license and in good standing with the College of Dietitian of Ontario

A 4-year Food and Nutrition or Nursing degree from an accredited university

Experience teaching/counselling individuals and groups of people

Must obtain and maintain professional liability insurance that meets the minimum requirement of the respective professional college.

Must obtain & maintain CDE designation

Experience working with individuals with obesity, Type 1 and 2 diabetes, and Freestyle LIBRE preferred

Must be licensed to practice in Saskatchewan, Manitoba, PEI (Quebec).
