What do you offer?


The Second Stage Housing Case Manager will work as part of the Housing Services Team that provides programs and services for individuals with affordable/social housing needs.

The Case Manager will work with the Director and Senior Director to achieve strategic goals in the fulfillment of the mission of the YMCA of the National Capital Region.


Some of the tasks of this position include:

  • Providing general information to individuals and agencies regarding eligibility for housing in the Y's Housing Services and specific information about the Second Stage Housing Program.
  • Scheduling and conducting interviews with housing applicants and their supports to assess eligibility and compatibility for the Second Stage Housing Program.
  • Developing and maintaining individual goal plans with assigned clients.
  • Providing case management, counselling support, crisis intervention, and referrals for clients.
  • Providing life and social skills workshops.
  • Maintaining standards for reporting and operating procedures.
  • Reporting statistical data.
  • Ensuring client confidentiality and that all files and records are secure.
  • Building and maintaining relationships with relevant agencies and services to promote referrals to the Second Stage Housing Program.
  • Maintaining the workplace to ensure a safe and clean environment.

What qualifications and abilities are we looking for?

Education

  • Relevant postsecondary degree, college diploma, or equivalent experience

Language

  • Fluent in English, oral and written
  • Bilingualism is an asset

Experience, Skills and Abilities

  • Minimum two years of experience in social services, preferably with youth
  • Knowledge and sensitivity in dealing with individuals with homelessness, addiction, and mental health
  • Knowledge of community resources relevant to youth
  • Current First Aid and CPR certification
  • Minimum one year of experience in the housing field preferred
  • Training in Suicide Intervention and Non-Violent Crisis Intervention preferred
  • Proficiency with Microsoft Office Suite
  • Proven facilitation skills for workshops and information sessions
  • Strong communication skills
  • Strong problemsolving skills
  • High degree of initiative and adaptability
  • Excellent client service skills and focus
