Position Summary


The Pharmacy Student will provide comprehensive admission medication reconciliation services to patients coming through the Emergency Department at Toronto General Hospital.


Responsibilities

  • Attend education and training sessions on the medication reconciliation process
  • Partner with the Toronto General Pharmacy team and patients to review all medications and non prescription medications on presentation to hospital.
  • Transcribe priortoadmission medications accurately into our health information system (Epic)

  • Experience working in a fastpaced healthcare setting
  • Increase knowledge of health care
  • Increase experience in audienceappropriate communication with patients and members of the patients care team

Qualifications/Competencies Required

  • Professionalism, ability to work with diverse group of healthcare stakeholders
  • Excellent attendance and highly reliable
  • Currently Enrolled in a Bachelor or PharmD Pharmacy Program
  • Strong interpersonal and patient relationship skills, with the ability to communicate effectively
  • Ability to work under pressure, solve problems and adapt to change
  • Demonstrate ability to work independently and also as a contributing team member
  • High level of flexibility to work days, weekends, evenings, and adapt to changing work and schedules
  • Experience working in healthcare, education, or customer service is an asset
  • Previous experience at UHN is an asset
  • Vaccines (COVID19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code
