Description:
As a Housing Case Plan Coordinator you will oversee the day to day operations of a peer home that houses 5 youth and maintain a staffing complement of four to six youth support staff.
You will be responsible for case management (completion of youth intakes, identify support needs and goals, develop holistic case plans, and facilitate discharges), youth advocacy & community engagement.
Case Plan Coordinators are expected to comply with industry standards and best practices while adhering all SCP mandated approaches.Qualifications (Education & Experience)
- Secondary completion required
- Post secondary strongly preferred (or experience equivalent)
- Minimum 2 year front line youth care experience
- Experience developing, implementing, and overseeing case planning & case management (Including but not limited to intakes, care plans, discharges, & transitions)
- Experience working with Ministry of Social Services, Child & Family Services, Community Resources, Indigenous populations, & Vulnerable populations,
- Experience supporting youth with exceptionalities, gang involvement, generational trauma/PTSD/CPTSD, experiencing current/previous exploitation, & managing substance use and/or mental health concerns.
Requirements (Skills & Competencies)

Valid Driver's License with Driver's Abstract:
Criminal Record Check with
Vulnerable Sector Clearance:- Cultural sensitivity required
- Demonstrated mastery of official SCP sanctioned approaches (Nurtured Heart Approach, Youth Centered Care, Harm Reduction, & Trauma Informed Care)
- Strongly developed interpersonal skills, emotional intelligence, & communication skills
- Well developed time management skills, organizational Skills, & attention to detail
- Preference will be given to individuals with CPR/First-Aid (B), Conflict Resolution, Crisis Intervention (DPVS), ASIST (Suicide Intervention), Mental Health First Aid & Water Safety
Duties & Responsibilities (Overview)
- Facilitate youth intakes and discharges
- Share information, communicate concerns, coordinate care with internal/external resources
- Develop, implement, oversee, review, update & communicate youth case plans
- Identify and bridge support/resource gaps
- Advocate for youth (i.e education, family, medical, mental health, substance use)
- Safety planning with youth
- Oversee the maintenance and upkeep of the peer home
- Crisis Intervention/management, rotating crisis line coverage (1 week on 5 weeks off)
- Facilitate and attend program/team meetings as required
- Comply with agency, program, and funder reporting requirements
- Ensure compliance with current agency policies, procedures, guidelines, and best practices
