The GIM TRANSITION COORDINATOR is ultimately responsible for working with medical teams to determine the expected date of discharge, to ensure that all of the necessary assessments, investigations and lab results are completed, and to ensure that the discharge proceeds on the scheduled date.

GIM Transition Coordinators will coordinate plans with community partners, and communicate directly with patients and families so they are well aware of the estimated discharge date and time.


Primary responsibilities include:

coordinating discharge and transition, planning to ensure that patients are safely discharged or transferred when appropriate; coordinating patient care, transition planning, assessments and investigations with General Internal Medicine (GIM) teams to achieve discharge from the hospital by 11am on the planned day of discharge; following discharge planning processes, ensuring patients have a daily care plan that is executed daily; facilitating investigations and treatments to move possible or probable discharges to confirmed discharges; implementing strategies to improve efficiency of patient discharge planning, ensuring appropriate patient education, following up and care planning to reduce the need for readmission; talking with patients and families to explore how to prevent future readmissions and how to coordinate better support in the home/community; tracking opportunities to improve time of discharge by evaluating delays in disposition decision making and identifying areas for improvement in expediting assessments, investigations and treatments.


QUALIFICATIONS:

  • Completion of a Bachelors degree in Nursing, Health Sciences or Health Administration
  • Basic Cardiac Life Support (B.C.L.S.) program
  • At least Three (3) to Four (4) years practical and related experience
  • Experience working in General Internal Medicine required
  • Membership in the relevant Ontario College of Health Regulated discipline
  • Current certificate of competency (licensure) from the College of Nurses of Ontario or a regulated Health Professional College
  • Demonstrates membership within a professional nursing association and/or nursing interest groups related to practice
  • Demonstrated strong leadership and motivational skills
  • Demonstrates ability to work in a team environment and collaborates with others in assisting with the delivery of care to patients
  • Demonstrated leadership abilities
  • Excellent skills in recognizing, analyzing and resolving problems
  • Excellent time management skills
  • Excellent decision making and problem solving skills
  • Clinical and patient teaching skills
  • Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards
  • Exercise initiative and good judgement with ability to multitask
  • Adapts to and implements change and facilitates its acceptance by others
  • Maintains and protects patient confidentiality in accordance with UHN Confidentiality policy requirements
  • Compliance with confidentiality requirements
  • Effective organizational, interpersonal, and communication skills
  • Knowledge of applicable legislative, UHN and/or departmental policies
  • Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
