    The OHT Implementation team under the Ontario Health Team (OHT) Strategy & Policy portfolio within the Population Health & Value Based Systems division at Ontario Health (OH) works closely with MoH, OH regions and OHTs to support the effective implementation of programs and initiatives to advance population health and integrated care.


    The Implementation team is looking for one (1) full time, permanent Senior Specialist, Implementation to join our team, to inform and support implementation of multi-faceted and innovative programs and initiatives to support OHTs to improve population health and achieve the quintuple aim.

    In 2019, Ontario Health Teams (OHTs) were introduced to provide a new way of organizing and delivering care that is more connected to people in their local communities, with improved access and better value for health care dollars.

    As a part of an OHT, health care providers (including hospitals, doctors, and home and community care providers) work as one coordinated team - no matter where they provide care.

    Our team works closely with the MoH, OH Regions, and internal and external partners to advance the maturity and implementation of the OHT model and oversees various programs including OHT performance and improvement, implementation supports and integrating strategic direction and new health service programming involving OHTs into implementation plans, including communications and engagement with stakeholders.


    The Senior Specialist will play a significant role on a cross-functional team, supporting initiatives to advance OHTs across the province.

    Working closely with the Manager and Leads, this role is primarily responsible for assisting with the delivery of program/operational mandates within a specific area of expertise by executing key activities to achieve required deliverables.

    As part of this role, the Senior Specialist develops and coordinates the required processes, tools, and technology to implement key activities and deliver program plans.


    Stakeholder engagement is a key component of this role, so we are seeking a strong team player who places high priority on collaboration, active listening and equity.

    We are looking for strong written, verbal, and visual/graphical communication skills, excellent problem-solving and critical thinking abilities.

    The ideal candidate has a proven ability to engage and partner with diverse stakeholders to embrace cross-disciplinary approaches and new ideas.

    Working knowledge of the fundamentals of quality improvement, implementation science, knowledge translation and/or change management, as well as experience using virtual collaboration tools and platforms (MS Teams, Zoom, SharePoint, etc.) are required.

    Fluency in French is considered a strong asset.

    Enable knowledge exchange and information sharing amongst a broad range of internal and external stakeholders, integrating feedback and data to inform program design and delivery

    Drive the conceptualization, development, implementation, and management of projects

    Support the Managers, Leads and other team members in the development, maintenance, and execution of work plans to ensure completion of deliverables on time, and escalate project risks as appropriate

    Develop and maintain productive working relationships with program area partners across cross-functional provincial and regional teams to design, plan, and execute work in support of program goals

    Develop and present materials for review and informational purposes using excellent written, verbal, and graphical communications skills, with ability to express complex concepts effectively

    Participate and lead, where appropriate, meetings with internal and external stakeholders to ensure consistency in work and identify opportunities for collaboration

    Lead project and program activities, including working with internal and external partners, leading advisory/working groups and project teams to operationalize and monitor tasks, and milestones

    Make recommendations with respect to related policies and procedures

    Develop various products including but not limited to briefing notes, analytical reports, executive summaries, status updates and presentations to support internal and external communication, business reporting and decision-making

    Conduct qualitative and quantitative analyses to support project and/or program goals, as required, presenting findings, reports and commentary as requested

    Work with staff from across Population Health & Value Based Systems, other branches of OH Provincial Office, Ontario Health Regions, OHTs and the Ministry of Health including administrative and clinical leadership, on cross-functional, collaborative projects

    Support coordination within Ontario Health and relevant MoH departments on related work, as appropriate, and supports other initiatives with the program area

    Education and Experience

    Bachelor's degree in Health Science, Health Administration, Business Administration, Data Management/Analytics, or related discipline

    Master's degree in in one of the above disciplines is an asset

    Advanced knowledge/skills and/or certification in implementation science, change management and/or quality improvement a strong asset

    Completion of equity, diversity and/or cultural sensitivity training required

    5 years of related work experience is required

    An understanding of Ontario's healthcare system with particular emphasis on chronic disease management, integrated care models, home care would be a strong asset

    Experience in the health care system, and more specifically working with government, OHTs, and/or community partners

    Experience engaging and working directly with health system stakeholders and health service providers; engaging patients, caregivers, First Nations, Inuit, Metis, and urban Indigenous and other equity-deserving populations required

    Expertise and experience in data analysis, literature searches, policy analysis, and business strategy, and the ability to translate information into a convincing written argument, summaries, briefs, or business case

    Experience supporting governance structures (e.g., steering committees, working groups, etc.) a strong asset

    Experience with collaborative tools/platforms to engage stakeholders (e.g., MS Teams, Zoom, SharePoint, etc.) required

    Fluency in French a strong asset

    Knowledge and Skills

    Strong service orientation with strong relationship-building skills with proven ability to develop relationships with key stakeholders to establish trust, credibility, and respect

    Ability to work independently and in a team, work under pressure and manage multiple tasks and deadlines, and navigate competing priorities and ambiguous deliverables

    Knowledge of Ontario's health care system and key system priorities, including Ontario Health Teams

    Strong ability to deal with ambiguity and flexibility to pivot to adapt to changing demands

    Knowledge of best practices related to inclusion and health equity required

    Excellent written, verbal, and visual/graphical communications skills, with ability to express complex concepts effectively

    Superior ability to analyze and define problems, evaluate alternatives, propose recommendations to support decision making

    Excellent organizational skills; responsible and highly motivated; must be able to manage multiple tasks, prioritize and effectively anticipate and respond to issues as they arise

    Detail-oriented and process-driven with a demonstrated ability to produce high quality work consistently and effectively

    Excellent skills in explaining complex concepts in a manner which is simple and easy to understand

    Strong problem-solving skills and demonstrated ability to think critically and strategically

    Strong computer skills including the MS Office suite; knowledge of MS Teams, SharePoint, PowerPoint, Excel as well as virtual collaboration/meeting platforms required (MS Teams, Zoom)
