Hours of Work Hourly visits and shift based care in the community (homes) throughout the day and evening (7am to 4pm OR 3pm to 11pm) Shifts up to 12 hours long during the day, evening or overnight (not sleep shifts)

Your role with clients
You will be working as part of a team with Nurses, Therapists, and other PSWs, and will deliver patient focused and high quality care in an integrated and team based way. On a regular basis, you will participate in team rounds/discussions (including the patient and family) to determine the patient’s progress towards goals, and changes to care plans. You will be a caring, responsible health care team member, who supports clients to be as safe and independent in their homes as possible, helping them to live their best lives. You will provide personal care to clients in their home, and may assist them to mobilize and perform light exercises. You will support caregivers to care for loved ones by providing direct care and respite. You will act at all times with the client and their family at the heart of all you do, observing, documenting, and reporting any changes in their condition to your Manager/Supervisor.
Client Profile Frail elderly population with complex care needs requiring support for ADLs and IADLs Clients may have cognitive impairments and responsive behaviours Respite services may be required for caregivers and care partners
Some of the activities you will participate in to support clients may include: Assisting with activities of daily living such as personal care needs, laundry, feeding, and other activities such as light housekeeping, meal preparation, reminders for meds, and other delegated activities, etc. Caring for patients with cognitive impairments and behavioural care needs Social interaction and supports for patients and caregivers Toileting and incontinence care/support Transfer support for patients (may be 1 or 2 person transfers or assists)
Skills & Requirements Strong verbal and written communication skills required; the ability to communicate in other languages is considered an asset Demonstrated ability to work as a member of a multi-disciplinary team, focused on the patient’s care needs and optimal care Comfortable with technology to support patient care and to facilitate communication with team members Flexibility and willingness to adjust schedules and work activities to meet patient care needs Ability and willingness to learn new program delivery model; new skills and activities as taught and delegated/assigned by regulated staff Ability to contribute in patient rounds as a member of the care team Highly reliable with demonstrated positive performance history and strong organizational skills Passion for delivering excellence in care for patients and their families/caregivers Availability during evenings, every second weekend and possibly overnight work Ability to travel from home to home within the community of Niagara Falls, Fort Erie, Port Colborne & Welland with own vehicle and driver's license Ability to stand, stood, bend, kneel, twist, walk and reach freely Ability to freely lift up to 35lbs/16 kg
What you will need to be successful Your Personal Support Worker Certificate or Equivalent, or valid certification as a Home Support Worker, Home Support worker level II, Home Support worker level III, Health Care aid or personal attendant Students currently enrolled (post first year) in health care programs such as nursing, rehab, para medicine or have a valid certificate as a Developmental Support Worker or a valid certificate as an RN , RPN, or Practical Nurse, midwife or physician ( either domestic or foreign –trained ) or RN students with a minimum successful completion of year 1 Current CPR and First Aid Certification A clear Criminal Record Check with Vulnerable Sector Screening within the last 12 months Proof of a negative TB test within the last 12 months and an up-to-date immunization record
