Vancouver Coastal Health is looking for a Registered Nurse to join the Home Health team at Central Community Health Centre as an Intake Nurse. Apply today to join our team!

As an Intake Nurse with Vancouver Coastal Health you will:

  • Conduct the centralized intake functions for the designated site/program services to ensure linkages between client and the appropriate resources occurs.
  • Perform initial screening, triage and referrals based on client needs, urgency of request and best practices to determine the appropriate treatment and educates and provides consultation to client/family.
  • Provide information to health providers and the general public, and establish and enhance relationships/communication within the designated program services and other community resources.
  • Prepare and maintain client records, maintain and provide accurate intake/referral data and statistics, and participate in team meetings and continuing education.
  • Engage in on-going team planning and quality improvement.

    QUALIFICATIONS:

    Education & Experience

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nursing Professionals (BCCNP).
  • Three (3) years recent related experience in the designated program area.

    • Knowledge & Abilities

  • Broad knowledge of nursing theory and practice within a client and family centred care model.
  • Broad knowledge of the British Columbia College of Nursing Professionals (BCCNP) standards of practice for nursing.
  • Broad knowledge of other facilities and community resources.
  • Broad knowledge of other health care disciplines and their role in client care.
  • Broad knowledge of adult education principles, methods and tools.
  • Physical ability to perform the duties of the position.
  • Demonstrated computer literacy to operate a computerized client care information system and word processing, spreadsheet, Internet and e-mail software.
