We currently have an exciting  casual  opportunity for you to bring your excellent work ethic as a dedicated  Social Worker  to our  Long Term Care Services  based out of  Peace Arch Hospital  in  White Rock, British Columbia.

Build on your education & career experience as you:

  • Provide therapeutic clinical counseling services to residents and families in the assigned clinical area within Residential Care.
  • Conduct social work and psycho-social assessments using a variety of modalities and in accordance with professional standards and clinical policies and practices and educate and provide consultation to residents, families and healthcare professionals.
  • Participate on the interdisciplinary team in care planning for the effective utilization of resident resources and participate as a member of local committees and planning bodies to identify areas for improved efficiency and effectiveness in service delivery to improve resident care services and reduce gaps in the community.
  • Provide education to resident/family/interdisciplinary team as needed regarding Social Work practice.
  • Communicate with community health and social agencies including government ministries, Continuing Care personnel and family physicians by collaborating in the assessment and management of individuals at risk, making referrals to appropriate resources when longer term treatment or follow-up is required and exchanging relevant information.

    • To join our team, you have:

  • A Bachelor’s degree in Social Work from an approved school of Social Work,
  • One year recent, related clinical experience.
  • Current full registration with the BC College of Social Workers.
