CASE MANAGER
The following is a job description for the Adult Employment Programs Case
Managers of the Bent Arrow Traditional Healing Society.

Ongoing - One-on-one counseling with students on their caseload which can include but not limited to;
Career coaching-moral support
Community referrals
Supported job search
Strategies to overcome barriers
Purchase bus passes for students, travel arrangements
Drive students to attain ID
Drive and purchase PPE for students
Drive students to interviews
Practice interview technics with students
Home visits
One-on-one Resume workshops when needed
Act as a role model for students
On the job visits
Goal setting with students
Advocate for students (with parole officers, court, income support worker etc.)
Build rapport with students under case load
Weekly contact by phone, in person or social media
Administrative Responsibilities;
Resume reviewing for your clients

Contacting clients weekly and uploading Case notes in the file
weekly
Assembling files
Faxing resumes to employers
Ensure payment authorizations are submitted
Purchasing safety tickets and setting students up for safety ticket training
Keeping files in cabinet in alphabetical order and tidy
Handing out bus tickets and tracking them
Boxing files and storing properly
Employer confirmations up to date in files
90 & 180 day follow up notes
Keep all company reporting/tracking forms up-to-date (Completed tracking &
Application status, Safety ticket tracking)
Ensure attendance records are kept up to date and filed
Interviews and Assessments with clients
Uploading all necessary documentation into our office 365 program
Keeping track of all expenditures and filling in Payment Authorizitions
Fill in Daily Staff Report online
Book out of office training for students and ensure all payment authorizations are completed and submitted to accounting
Provide a monthly report to the supervisor compiled of the following information:
# of students accepted into Journey to Success per class
# of students suspended and no show for first day of class
# of clients employed - full time, part time
# of clients who found employment before end date of class portion of JTS
# of clients employed at 90 and 180 day follow ups
Current program starts date and # of students who started and currently continuing in the program
Track # of students that access bus tickets
Mobius;
Input starters
Input completers
Input 90 & 180 day follow ups
Check students Mobius notes as needed
Add notes is Mobius if client is not attached to the program

Cover off for;
Employment resource room
Class room workshops
Computer lab-safety tickets, resume workshop
Other positions as directed by Team Lead
The Northeast Hub
Weekly Orientation; ( this is done when we have class at the office)
Provide community awareness about Bent Arrow program, current Employment program information, and ensure you are marketing the program.
Solicit the names of businesses that are interested in hiring the students who complete the program and generate a list to be submitted to the program manager.
Any other duties as requested by the senior manager or team lead.

$25.00-$27.00 per hour
