    Under the supervision of a provider, lead all aspects of care management for patients with chronic disease and annual preventative screening diagnostics and care
    Demonstrate comfort with providing in-person and virtual care
    coordinate patient flow with virtual encounters in preparation for the visit; gather and document pertinent data (vitals, medications, allergies) from the patient and enter information into the medical record
    Maintain complete and accurate patient records by documenting chief complaints and conditions, verifying medical history, allergies, medications, vitals, and all patient interactions and treatments administered
    Perform basic procedures and tests, including venipuncture
    Manage quality measure requirements for CMS, including but not limited to HEDIS, adult preventative, and STARS
    Manage risk adjustment (HCC Coding) for a panel of members with in a specific care team
    Set up patient rooms for mobile clinic visits, set up necessary equipment, instruments, and supplies, and clean as appropriate
    Registered Nurse degree from an accredited school of nursing
    Licensed/eligible to practice in the United States
    Passion for delivering care in rural America
    Experience with medical or health data is a plus
