Unit Summary:

THP is a Regional Cardiac Program providing services and care in collaboration with our Regional Partners. 2C Cardiology is a 50-bed inpatient, acute care unit.

We support a wide variety of complex cardiac patients, while working closely with Cardiac Cath Lab, Cardiac Short Stay and Coronary Care Unit.

We primarily focus on medical management and mínimally invasive procedures to care for our patients in their unique care needs.

We also support off-service patients with telemetry needs.

We have a broad team of skilled, interdisciplinary professionals to support our patients, and ensure they receive the best care possible while in our care.


Position Summary:


Enhance the overall patient & family experience by coordinating and facilitating the cardiac patient care team processes to meet the care needs and achieve quality clinical outcomes for all patients.


Responsibilities:


  • Your leadership, facilitation, decision making, conflict resolution and clinical abilities will promote and encourage staff empowerment
  • Plans, organizes and coordinates the day to day activities within the Cardiology units
  • Collaboratively works with Clinical Quality Care Leaders in CSSU and CCU & CVU to facilitate patient flow
  • Uses riskbased guidance tool for management of PUI patients & cohorting to create capacity on the unit
  • Collaboratively works within Cardiology units to create capacity and pull patients up from the ER/CCU as per guidelines
  • Provides clinical support to staff and patients
  • Identifies gaps in service and makes recommendations to address issues
  • Implements and evaluates educational programs/strategies that are evidence based and support best practice in the management of patient care
  • Acts as a link with the Clinical Educator(s) and support the implementation of new educational initiatives for staff
  • Consults and facilitates with community agencies if required
  • Participates in the recruitment, retention and evaluation of staff
  • Monitors quality indicators including patient satisfaction

Key Qualifications:


  • Graduate of a recognized College or School of Nursing and preferably hold a Baccalaureate degree
  • Registered in good standing with the College of Nurses of Ontario (CNO) or currently meet the eligibility criteria to register in good standing in the Temporary Class or General Class with the CNO
  • ACLS Certified
  • Advanced Clinical Preparation with a minimum of 5 years of sound clinical experience in Cardiology
  • Previous leadership experience preferred
  • Strong communication skills along with the ability to establish positive relationships/partnerships with all staff, patients, families, internal and external partners
  • Demonstrates initiative, creativity and innovation in clinical practice
  • Excellent critical thinking and decision making skills
  • A demonstrated commitment to patient centered care
  • Ability to promote change and support and mentor others through the change process
  • Demonstrates continuous learning and professional development
  • Welldeveloped computer skills
