In keeping with the FACT standards, a comfort in providing assertive outreach, engagement, home visits and community-based care is a must. This position reports to the Manager, Adult Intensive Services.

Flexible Assertive Community Treatment (FACT) is a recovery-oriented, evidence-based best practice.

A FACT team is comprised of a multidisciplinary team of clinicians monitoring and treating clients with severe and persistent mental illness (SPMI).

The target client population is those who require both assertive community treatment (ACT) with a larger majority of clients on the team who need less intensive treatment and support and could be served in a traditional intensive case management model.

To combine care for these two levels of care, the FACT team employs a flexible switching system where the intensity of services can be ramped up or ramped down as needed depending on individual client need.
