This position provides nursing services and interventions to residents based on assessments and care planning procedures, and within the LPN scope of practice.
The Challenge for You
Amongst the many other day-to-day responsibilities, some of your duties will include:
Assesses and monitors resident and family actual and potential strengths and limitations including physiological, psychological, socio-cultural and spiritual needs.
Where appropriate encourages elder participation in activities of daily living.Provides direct nursing care, such as assisting with toileting, bathing, feeding, dressing, grooming, oral hygiene and care of skin/nails.
Also, performs medical treatments and procedures such as catheterizations, dressings (sterile and non-sterile), administration of medications, blood pressures and temperature/pulse/respirations.
Assists other team members in providing care.Receives physicians orders, arranges for treatment, medication and other therapeutic services as needed.
Develops appropriate nursing care plans in conjunction with interdisciplinary team and sets goals with resident in relation to problems/needs as identified through assessments.
Evaluates care plans and makes changes when necessary. Attends and contributes to care planning meetings and family care conferences.Records and maintains resident health record according to established format and facility guidelines.
Records (changes, observations as per facility guidelines/format, and reports any concerns in residents condition beyond LPN scope such as mobility, temperature, skin integrity, appetite and/or behavior) to Nurse-in-Charge.
Participates in Quality Assurance/Risk Management Programs by completing documentation in accordance with established procedures, participates in audit activities by compiling information/preparing statistics, as requiredProvides guidance to the Care Aides under the direction of the Registered Nurse.
Admits, discharges, and/or transfers residents according to facility policies as required, and completes necessary forms.
Monitors use of equipment/supplies and reports malfunctioning equipment and supply needs to appropriate staff.
Contributes to the development and modification of resident care policies and procedures, as well as those which pertain to safety of residents and staff.
Participates in medical, departmental and general staff meetings, which include interdisciplinary team conferences and committees as requested and provides input that relate to resident care and the operation of Haro Park.
Provides emergency-response care until RN arrives in nursing unit.Works with staff and residents to promote safety.
Work is frequently performed under a variety of working conditions which may include exposure to hazards, disease and inclement weather.

Your Education and Experience Should Look like This
Graduation from a recognized program for Licensed Practical Nurse or an equivalent combination of education, training and experience. Current full practicing licensure with the British Columbia College of Nurses and Midwives. (BCCNM).
Sensitivity and a passion for meeting the needs of older adults and an ability to interact with residents, families and members of the community.
Ability to communicate effectively both verbally and in writing.Ability to deal with others effectively.
Physical ability to carry out duties of the position.
Ability to organize work.
Ability to operate related equipment.
