We are seeking:


Intake Social Worker

Permanent Full-Time position (35h/week)

(Hybrid 1 to 2 days/week)

_What will be your role?_

  • Reporting to the Senior Manager, Community Support Programs, this position is responsible to perform all intake functions for the Community Support Programs including programs service coordination and support: receive, assess, and process all requests for service from potential clients and referral sources, provide information regarding the criteria for the programs and range of services offered and refer to other community services when ineligible. Participates in VHA services and quality initiatives. All activities are conducted within the policies, procedures, and standards of confidentiality of VHA._

_ Key tasks and responsibilities of the role:_

  • Receive, assess, and process service requests from potential clients and referrals
  • Conduct a thorough assessment to identify the community program best suited to the client's needs including a financial assessment to determine program eligibility
  • Ongoing assessment and case planning through a coordinated team approach within VHA and referral sources
  • Promotion, outreach and linkage to community resources
  • Monitoring, tracking and identifying trends and service gaps
  • Administrative, planning and scheduling support
  • Participation in service, program development and quality initiatives

_Desired Qualifications:
_


  • Bachelor's degree or diploma in related field: BSW or Social Services diploma combined with a minimum of 2 years' experience
  • Registered with Ontario College Social Worker and Social Service Workers (OCSSSW)
  • 1 year of relevant intake assessments experience
  • Knowledgeable in working with a range intake assessment tools, formulation, and case planning skills
  • Excellent solution focus and conflict resolution skills with a clientcentered approach and crisis intervention skills
  • Strong knowledge of case management theory, principles, and practices
  • Knowledge of risk and family assessment framework, methods, and techniques
  • Strong interpersonal and communication skills with comfort working in a high pace team
  • Strong analytical skills, combined with excellent written skills for documentation and assessment
  • Experience with project initiatives and evaluations at asset
  • Culturally competent and able to interact effectively with diverse stakeholders
  • Ability to handle a high volume of telephone calls and inquiries in an engaging manner
  • Strong planning, prioritizing, and excellent organizational skills
  • Knowledge of the GTA area and the transportation routes
  • Satisfactory completion of technical skills testing is required
  • Working knowledge of Gold Care is a definite asset
