Hillcrest Reactivation Centre is a 75-bed community based reactivation Centre, part of the Community Transitional Care Programs within the TCLHIN.

Operating under the Home and Community Care Act, Hillcrest offers a short period of reactivation to clients who are ALC or at risk of ALC, supporting their return home.


Job Summary:

In the provision of patient and non-patient care activities, the primary responsibilities of the Social Worker
include:

  • Providing comprehensive psychosocial assessments and intervention
  • Providing educational and supportive counselling services; providing service based on individual, family and group practice models
  • Discharging planning, population health advocacy, community services linkage
  • Collaborating with the interprofessional team and communitybased service providers
  • Advocating, mediating and resolving conflict
  • Participating in professional, program and hospital development
  • Willingness to provide field practicum Instruction to Social Work students
  • Participating in current and future program development, quality initiatives and research activities
  • Complying with, and commitment to, patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients.

Qualifications:


  • Masters degree in Social Work required
  • Registered member of the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Member of the Ontario Association of Social Workers preferred
  • Experience working with adults with complex medical needs
  • Excellent time management, organizational, interpersonal and communication skills
  • Excellent skills in recognizing, analyzing and resolving problems
  • Computer proficiency required
  • Knowledge of the use of selfassessment measures preferred
