Client Care Manager
I Have A Chance Support Services Ltd. strives to support and encourage people on a route to independence. Our enthusiastic and creative team is looking for a CLIENT CARE MANAGER who is passionate about helping others.


ROLE

To support individuals with disabilities and complex needs within the designated Support Homes and IHACSS programs to achieve home living, life skills, daily living, and community inclusion goals.
To practice and promote the expectations and responsibilities of their position:

To be a positive role model and advocate for individuals receiving services from I Have A Chance Support Services Ltd.


To support individuals to be safe within their home and in the community and to identify and deal with any concerns.


To support individuals to understand their personal rights, the rights of others, how their behaviour impacts others, and how to identify and report situations of suspected abuse.

Maintain a working knowledge of each individual and establish healthy boundaries and professional relationships based on respect and dignity.

Support individuals in times of crisis and challenging behaviours

To support employees to achieve success within their role

To role-model IHACSS expectations and guide employees to follow expectations, policy and procedures

To establish and maintain professional rapport with funders, stakeholders and government contacts

To develop individualized programs and track successes. Ensure reporting is up to date, maintained and submitted to stakeholders appropriately

To provide coaching and skill development for employees and individuals in care

To ensure Client Care Managers are maintaining appointments and upholding reporting requirements

To ensure individual in care files meet contractual and accreditation standards


RESPONSIBILITIES

SUPPORTING INDIVIDUAL IN CARE PROGRAMS AND WELLNESS

  • Maintain a working knowledge of each individual in care and a professional relationship based on respect and dignity
  • Ensure employees are providing a support services to individuals with respect to maintaining a household, personal health, developing relationships, managing finances, active community inclusion and designated programs

This may include:
Meal planning, preparation, household cleaning, transportation and other duties as required

  • To support individuals to understand their personal rights, the rights of others, how their behaviour impacts others, and how to identify and report situations of suspected abuse
  • To support an individual's freedom of belief, faith, religion, and culture and assist individuals to practice these as desired
  • To support individuals in the development of healthy and positive relationships
  • To support community inclusion and to assist individuals to become active, valued, and contributing members of the community
  • Promote and develop individual in care safety and wellbeing practices
  • Role model appropriate and healthy behaviours and practices while on shift
  • Ensure all medical appointments are followed through and documented accordingly
  • Provide afterhours On-Call Support as outlined in IHACSS policy
  • Develop a professional relationship with individuals in care through relationship building and active engagement regularly
  • Report to the Support Homes regularly to ensure all IHACSS policy, procedures and expectations are being followed
  • Provide rolemodeling to individuals in care in regard to healthy daily living skills
  • Create and maintain individual in care files
  • Provide legal guardians and stakeholders with timely, profession and accurate communications
  • Manage intake and discharge processes
  • Attend scheduled individual in care related meetings in lieu of Client Care Manager as assigned by the Program Manager
  • Back fill Client Care Manager vacancies as needed
  • Manage IHACSS Medication Consents, Intake Consents, Annual Consents and as needed consents for each individual in care
  • Document and record all team meeting minutes for the Client Care Department
  • Provide phone call management and appointment support to Client Care Managers
  • Assist the IHACSS Administrative Assistant in reception support as needed

REPORTING

  • Participate in drafting, editing and updating Quarterly Reports as assigned
  • Ensure Support Homes are creating and document Behaviour Tracks, Positive Occurrence Reports and Unusual Occurrence Reports as required
  • Ensure accurate and timely completion of all contractual documentation
  • Maintain accurate, appropriate, specific and timely reporting in all documents
  • Ensure all Individual in Care files are maintained, accurate and uptodate
  • Ensure completion and accuracy of all required documentation (e.g. Program Participation Update, Task Sheets, Suicide Risk Assessments and watch logs, Consultation Forms, Unusual Occurrence Reports, etc.)
  • Provide recommendations and referrals to internal and external agencies, activies
