What You Will Do:
As a full time, permanent member of the Service Planning Team, you will work with families and community service partners to understand the strengths and needs of the client and family to enable the development of a holistic and coordinated plan.
You will provide information in response to identified goals, and link the client and family to appropriate services in the community.
You will use critical thinking skills to respond to and resolve issues in collaboration with the client, family and community partner agencies.
This position reports to the Manager - Family and Community Supports.Responsibilities:
- Deliver service in a comprehensive and flexible manner in order to act as primary point of contact for children and youth and their family to navigate and access services to meet the goals for their child.
- Support the development, monitoring and updating of a service plan including, but not limited to:
- Ensuring planning is collaborative and demonstrates an inter-agency, inter-professional, and cross-sectoral approach,
- Working collaboratively with families and their formal and informal supports to facilitate the development of planning goals, strategies and activities that are based on the priorities of the family,
- Analyzing the nature and level of need as well as strengths to inform goals and decisionmaking, and
- Facilitating meetings, monitoring the delivery of service and negotiating with agencies on behalf of the family.
- Provide service coordination to families and community agencies including but not limited to:
- providing resource materials and information,
- working collaboratively with families and their formal and informal supports.
- Collect comprehensive information that supports service delivery at the beginning and throughout the family's participation in the service.
- Work collaboratively with community partners including providing leadership in the concept of a team approach to service.
- Act as a liaison with the community at large.
Qualifications:
- Undergraduate degree in social sciences or related field. Diploma in relevant field with experience also considered.
- Minimum of 5 years' experience in the children's service system.
- Extensive knowledge of the range of support services available to children and their families in Durham Region and beyond.
- Ability to assess needs of client and make sound judgments to coordinate activities.
- Demonstrated ability to effectively support families experiencing significant emotional distress and identify risk situations requiring prompt action.
- Exceptional leadership, team facilitation, conflict management, negotiation, and mediation skills that support the development and maintenance of good working relationships.
- Good case management/service coordination and organizational skills to support planning for client services.
- Ability to work independently and participate as a team member.
- Strong communication skills, written, oral, listening and nonverbal.
- Willingness to work flexible hours as required.
