The Opportunity:
We are looking for a
Transitions Manager with strong leadership skills to guide our dynamic team at
Connecting Care Inc.
Skills and key deliverables that will take you further:

building a team for success- Exhibits respect, patience, and collaboration with your team
- You have a Project Management background and pay attention to detail
- Commitment to Person Directed Care approaches (familiarity with the Eden Alternative Philosophy and the Domains of Well-Being is preferred)
- Able to coordinate within the vision, objectives, and guiding philosophy of Connecting Care
- Collaborate with ease with several internal and external stakeholders
- Demonstrates excellence in building partnerships with Residents, family members, various care partners to ensure the vibrancy of our community
- Supports in the roll out and compliance of Well-Being Strategies
- Maintains the provision of highquality services needed and/or desired by Residents
- Accountable for determining recruitment needs for communities
- Orientate General Manager's to attain/maintain key measurable indicators, targeted occupancy levels and overall daytoday tasks
- Mentor to align expected project schedules
- Ensure an interdisciplinary effort to achieve the successful integration of quality of life and quality of care
- Flexible, willingness to prioritize and readjust routines, open to change for the benefit of the community
- Strong attention to detail, organization, communication, and time management skills
- Ability to multitask and shift priorities throughout the day
- Excellent problem solving and conflict resolution skills, deal effectively with difficult or crisis situations
- Ensure that care delivered recognizes each Resident as an individual
- Knowledge of supportive living services, assessment, and care planning
- Maintains ongoing compliance with appropriate local, provincial, and federal regulatory standards
- Ongoing monitoring and continuous quality improvement
- Demonstrate awareness of accident and injury prevention; adheres to safe work practices and procedures
- Flexible for travel of up to 75%, overnight stays for periods of time
Foundational knowledge and experience to grow from:
- Post-Secondary diploma or degree in Business Administration, Management or Health Services required
- 3+ years' experience management experience within nursing, recreation, or hospitality preferred
- Proven leadership skills in supporting a team to success
- A 'PMP' or 'CAPM' Project Management Designation would be an asset
- Excellent communication skills
