Job summary
This position will provide casework and support to residents of Connective's programs to enhance their quality of life, to promote independence, to ensure the orderly and safe operation of the facility.

- Participates in the assessment, goal setting, and program planning - such as Personal Service Plans - for residents
- Evaluates client needs and develops short-term plans to meet such needs with the active participation of residents and their families
