About the Role:


The Supportive Housing Portfolio provides long-term, affordable housing solutions with flexible and voluntary support services offered to support high-acuity individuals who are homeless with housing retention and independent living.

This Counsellor role is dedicated to serving tenants of the STC Supportive Housing Portfolio in conjunction with other program staff.


Role Type:
New Full time, Permanent


Hours of Work - Days, with some evening and weekend work required as part of the weekly schedule, on a rotational basis.

Qualifications:


  • Completion of a BSW or other University Degree relevant to mental health, addiction or Social Services
  • Minimum one year of progressive experience in addictions or mental health; experience in concurrent disorders or trauma informed practice an asset
  • Previous experience in housing preferred
  • Registration with a professional college (i.e. OCSWSSW or CRPO) required
  • Lived experience related to addictions and/or mental health an asset
  • Previous experience in housing preferred
; experience with rural populations an asset

  • Experience within the homelessness serving sector an asset
  • Lived experience related to addictions and/or mental health an asset
  • CSC Clearance conducted by PWGS Canada
  • Standard First Aid/CPR, Level C
  • Valid G Driver's License (G2 or G) and Satisfactory Motor Vehicle Record
  • Reliable vehicle and willing to transport program participants
  • Must provide proof from Insurance provider of 1) at least $2 million in general liability and 2) acknowledgement of scope of vehicle use by Insurance provider

Role Overview:

Counselling Services

  • Support the intake of clients through consultation and assessment
  • Deliver clinical and support services for clients of the program including:
  • Develop goals and monitor progress towards them in collaboration with program participants and identified supports (where appropriate)
  • 1:1 counselling and psychoeducational interventions that support progress towards identified goals and enhance overall health and wellbeing, particularly in relation to mental health and addiction
  • Facilitate house meetings, psychoeducational and support groups
  • Crisis planning and intervention as required
  • Utilize evidenceinformed practices including (but not limited to) Motivational Interviewing, Stage-Matched Intervention, Trauma-Informed approaches, CBT and DBT
  • Support to maintain housing and develop employment/education, budgeting and life skills
  • Make referrals to internal and / or external program or services, as appropriate
  • Comprehensive and collaborative discharge planning
  • Facilitate community building with program participants and the Wellington and Guelph communities including neighbors, community groups, resources, etc.
  • Participate in program development activities as required
Consultation and Capacity Building

  • Provide clinical consultation to SAMH Intensive Case Managers/other SAMH team members Work effectively and in collaboration with the broader Supportive Addiction & Mental Health Housing Team including staff of CMHAWW, as well as external service providers, landlords, the family/support networks of participants, and other stakeholders
Administration and Reporting

  • Maintain appropriate confidentiality and documentation standards as required by the agency
  • Involvement in program evaluation, data collection/input into CaseWORKS
