Summary of Job:


The Thrive Program Case Manager will provide client-centered service focusing on expecting or new mothers who are struggling with substance use in the Lanark, Leeds, and Grenville areas.

The role will encompass leading psychoeducational groups, individual supportive counseling, connecting clients with services, general case management, collaboration with community partners, and advocating for clients.

The Case Manager's home office will be in Brockville, Ontario and will include travel to Prescott, Gananoque, Perth and Smiths Falls.


Qualifications and Responsibilities:


  • Bachelor of Social Work or equivalent from a recognized institution.
  • Registered with the College of Social Workers and Social Service Workers of Ontario or the College of Registered Psychotherapists of Ontario.
  • Two years' experience working in Mental Health and Addictions, with specific experience in substance use counselling, harm reduction, and abstinence models of care.
  • Working knowledge of the Stages of Change and Motivational Interviewing.
  • Current First Aid and CPR Certification required.
  • Ability to work independently and also as part of a multidisciplinary team.
  • Experience with leading groups and individual counseling sessions.
  • Excellent communication, interpersonal, and conflict resolution skills.
  • Strong problemsolving abilities.
  • A high level of independent decision making skills.
  • Ability to adapt to changing environments and manage time effectively; willingness to be flexible and facilitate change.
  • Demonstrated crisis intervention skills/experience and ability to respond to crisis situations as they arise.
  • Aboveaverage computer and keyboarding skills.
  • A valid Ontario Driver's Licence, access to a vehicle, and vehicle insurance with a minimum of $2,000,000 liability.
  • Satisfactory Vulnerable Sector police records check required.
  • Ability to work within the Mental Health Act, Occupational Health & Safety Act, Ministry of Health and Long-Term Care guidelines, Psychosocial Rehabilitation Practitioner's Code of Ethics, and Best Practices.
  • The Upper Canada Family Health Team has a Mandatory Vaccination Policy which requires all employees to be fully vaccinated against COVID19 and provide proof of vaccination as a condition of employment

Salary:
$32.02 per hour

Expected hours: 37.5 per week


Schedule:

  • Day shift
  • Evening shift
  • Monday to Friday

  • Brockville, ON K6V 5J9: reliably commute or plan to relocate before starting work (required)
