_ Program Overview - Youth Wellness _

  • WoodGreen is committed to supporting youth. The Youth Wellness Program provides youth with support services including mental health supports, employment, transition support from the child welfare system, and other services that support young people as they navigate the journey towards adulthood._

_What You Will Do_
- _

Operate from a critical, trauma-informed lens, including Anti-Black Racism (ABR), Anti-Oppressive practice (AOP), and demonstrated understanding of colonialism and its intergenerational effects on black, racialized and Indigenous communities.

_
- _ Use professional case management practices and a variety of clinical modalities to perform case work duties and prioritize needs, which include assessment, short and long-term goal setting and the development of action plans with clients, ongoing intervention, re-assessment and case termination for youth between the ages of 13-30._
- _ Provide onsite supportive and clinical counselling, guidance, direction, and referral pathways to youth experiencing mental health issues and concerns, including but not limited to youth experiencing crisis in need of immediate intervention._
- _ Provide onsite follow up care planning and service coordination as well as advocacy._
- _ Keep updated information on WoodGreen and external services & activities including eligibility criteria, program information and contact details._
- _ Support various drop-in and event-based programming for youth as needed, including but not limited to Housing 101, Rent Smart and other Youth Wellness Centre (YWC) drop in activities._
- _ Maintain detailed client files and case notes using Pirouette, Customer Relations Management Systems (CRM
  • Microsoft Dynamics) or other applicable software and maintain regularly._
- _ Attend monthly supervisions with management to audit case files and receive direction, feedback and frontline support._
- _ Participate in staff meetings and clinical consultations as needed._
- _ Help to create reports, capture data, tracks trends and activities, and work with internal and external program staff and managers to improve client experience over time and respond to emerging client needs._
- _ Attend committee meetings to collaborate with agency partners as needed._

**_
What You Bring to the Team_**- _
Master's Degree in Social Work_
- _ Registration in good standing with the Ontario College of Social Workers and Social Service Workers, or College of Registered Psychotherapists of Ontario, or an equivalent._
- _ 3+ years of experience in clinical case management preferably with youth clients._
- _ 3+ years of experience with individuals who have experienced homelessness, violence or other related issues at a case management level, and employment planning and preparation work._

Experience in a variety of counseling modalities especially related to working with youth who have experienced homelessness and violence by an intimate partner.

- _ Strong computer skills, including internet research skills._
- _ Knowledge and skills in the use of social resources._
- _ Organizational, planning, and time management skills. _
- _Interpersonal skill and ability to work in a team setting._
- _ Knowledge in community development._
- _ Able to work under stress._
- _ Knowledge and sensitivity to issues of culturally diverse clientele._
