Position Summary
As an integral member of Allied Health, the Discharge Service Coordinator will:

Duties

  • Communicate effectively with referral sources, ensuring excellent customer service;
  • Discharge, plan and coordinate patient discharges in close collaboration with the interprofessional team and Program Services Manager;
  • Manage waiting lists and communicate with patients/families on a regular basis;
  • Coordinate/prioritize services across the care continuum, that is, inpatient to outpatient, between streams, etc.
  • Demonstrate flexibility in dealing with competing priorities on a daily basis;
  • Maintain uptodate census information and coordinate/chair rounds, patient/family conferences, and team meetings related to discharge; maintain a patient focused environment;
- coordinate quality activities, data collection, and evaluations; monitor performance indicators and generate reports; demonstrate an understanding of, compliance with, and commitment to, patient safety responsibilities and corresponding hospital plans policies and procedures to ensure a safe environment for patients.


Qualifications

  • Bachelor's degree with a clinical background, i.e. health related field, required.
  • Member in good standing with respective college.
Experience in a healthcare environment required.

  • Experience in coordinating discharges and/or patient flow preferred.
  • Knowledge of standards of practice and applicable health care legislation.
  • Knowledge of resources available for patients returning to the community.
  • Ability to network and nurture relationships within the healthcare industry.
  • Knowledge of quality/performance improvement theory and practice required.
  • Leadership abilities required.
  • Excellent computer skills required (Word and Excel required).
  • Excellent verbal and written communication skills required.
  • Excellent interpersonal skills.
  • Self directed individual with the ability to work both independently and in a group setting.
  • Must be proficient in English.
  • Excellent attendance required.
