QUALIFICATIONS:
  • Bachelor's degree in a healthcare related field with active registration/membership in a health or health related professional college
  • Must be a member in good standing of a respective regulatory College (COTO, CNO, OCSWSSW) under the Regulated Health Profession Act, 199
  • Current license/registration in related field
  • Minimum of two years experience in Clinical Hospital environment o Minimum two years of recent experience in case management, utilization management, discharge planning
  • Understanding of Alternate Level of Care (ALC,) patient flow processes.
  • Experience in collaborative care planning within hospital and community setting
  • Demonstrated knowledge of medical terminology, ability to understand patient care plans, pathways and treatment plans.
  • Demonstrated knowledge of coordinating and leading discharge planning with the interdisciplinary care teams and patients and their families.
  • Demonstrated understanding of financial and personal care capacity, and the ability to consider capacity for the purposes of Discharge Planning
  • Comprehensive and demonstrated knowledge of community resources including congregate care, housing, community supports services, and social service agencies
  • Evidence of effective organizational, interpersonal, communication, leadership, judgment and decisionmaking skills
  • Applies Niagara Health values and professional practice standards to patient/client care and service
  • Demonstrated commitment to patient/client centred care
  • Demonstrated commitment to collaborative planning and relationship building
  • Applies skills by providing, facilitating and promoting quality patient care within an interprofessional team
  • Demonstrated commitment to patient and family education and the ability to promote health and wellness
  • Demonstrated commitment to patient safety and safe transitions in care
  • Integrates education, critical thinking, and relevant practical experience in problem solving
  • Consistent and regular attendance mandatory
  • Proficiency in both official languages (English and French) will be considered an asset
  • Vaccines (COVID19 and others) are a requirement of the job unless you have an exemption based on medical or on a ground pursuant to the Ontario Human Rights Code
