Your Opportunity:
As a Single Independent Living Support Worker (ILS) you will be part of an interdisciplinary team, assisting clients to develop and maintain skills as identified in the treatment plan associated with living independently in the community.
The ILS worker assists the designated health care professional (HCP) with the implementation of the client's treatment plan by providing goal oriented independently living supports.
The ILS worker relays factual, timely information to the HCP which assists in the evaluation and refinement of the overall treatment plan.
This position will support the Addiction & Mental Health Adult programs in Sherwood Park and Fort Saskatchewan.Description:
As a Single Independent Living Support (ILS) worker, you will be part of an interprofessional team, assisting patients (client, resident) to develop and maintain skills as identified in the care plan, associated with living independently in the community.
You will provide support with preventative health care, health maintenance, quality of life, life skills coaching and connect patients to appropriate community supports.
You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:
Completion of Grade 12 or equivalent.
Additional Required Qualifications:
Two years' experience working with persons with addiction and mental health concerns. CPR Basic Life Support. Nonviolent Crisis Intervention. Experience working in a community based setting. Experience with an Electronic Medical Record.
Preferred Qualifications:
Social Work diploma or Mental health aide certificate preferred. Equivalencies will be considered.
