As the client base continue to grow, this position will take a leadership role in supervising a growing team of companions, homemakers, and personal care workers.

Responsibilities:
As the Home Health Service Specialist, your responsibilities will include, but are not limited to:
A
Services Provided:
- Provide direct client care as needed in a variety of roles such as:
Personal Care:
Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, medication reminders, toileting, and mobility.

Homemaker Assistance:
Help clients with household tasks such as meal preparation, light cleaning, laundry, and grocery shopping. Ensure a safe and clean-living environment.

Emotional Support:
Offer companionship and emotional support to clients. Engage in conversation, provide a friendly presence, and help alleviate feelings of loneliness or isolation.

Transportation:
Provide transportation for clients to medical appointments, social outings, or other necessary errands.

Day Program:
Support and provide assistance with our on-site Day Program, which offers a range of group activities and support services tailored to the needs of our senior participants.
- Stay current with best practices in home care, attend training sessions, and pursue professional development to provide the highest level of care.
- Respect and adapt to the cultural and individual needs of each client to provide personalized care.
B.
Supervision and Mentorship
- As the business grows, this role will lead and supervise a team of companions, homemakers, and personal care workers, providing guidance, training, and support.
- Collaborate with management to participate in the recruitment, selection, and training of new companions, homemakers, and personal care workers.
- Help onboard new team members, ensuring they receive proper training and orientation.
- Conduct regular team meetings to discuss client care plans, best practices, and address any concerns or questions.
- Conduct supervisory shifts with employees for onthejob training and periodic performance appraisals.
- Ensure continuity of care for clients during staffing transitions and while the team is growing.
- Complete first visit with a new client to understand firsthand what is expected and fill in shifts as needed.
- Implement safety measures and precautions to prevent accidents or injuries in the client's home. This may include fall prevention, infection control, and emergency response protocols.
- Ensure compliance with all relevant laws and regulations governing home care services, including HIPAA (Health Insurance Portability and Accountability Act) for patient confidentiality.
- Demonstrate a caring and empathetic attitude towards clients, understanding the challenges they may face due to illness or disability.

Documentation
- Collaborate in the creation and maintenance of detailed documentation of roles, responsibilities, and expectations for companions, homemakers, and personal care workers.
- Document clientspecific care plans, scenarios, and any modifications made during the care process.
- Maintain accurate records of client progress, staff performance, and incident reports.
- Utilizing the online database, ensuring client information is accurate and being used properly.
- Provide training on platform to caregivers, answering any questions about charting or documentation.
D.
Idea and Work Plan Development:
- Collaborate with management and team members to develop creative ideas and work plans to meet the goals and objectives of our home care services.
