Reporting to the Primary Health Coordinator, you will be responsible for:Participate in Health Centre community health, treatment and surveillance programs and activities Acts independently and as a member of a multidisciplinary team to promote, plan, implement and support activities, programs, and client/community care Responsible for liaising between clients, families, the community and health care providers to educate, advocate, and support holistic wellness Job Qualifications Community Health Representative Certificate/Diploma (opportunity for training)
