The Canadian Red Cross (CRC), a member of the largest humanitarian network in the world, is currently seeking a Case Worker, Community Health.

Reporting to the Manager, Operations and Development, and in close cooperation with members of the Quebec Community Health Directorate team, the Case Worker, Community Health will be responsible for assessing the needs of older adults based on the collected information, determining a service trajectory based on their needs, proposing a personalized intervention plan, and making the appropriate referrals, in accordance with the Fundamental Principles of the CRC and the department.

Conditions: This is a temporary full-time position (35 hours a week) with the possibility of extension.


Responsibilities

Case management:


  • Analyze and assess the needs of previously identified clients.
  • Communicate with these people to develop a support plan.
  • Liaise with the appropriate community organizations to implement the support plan.
  • Ensure that feedback is received from beneficiaries about the services they obtained.
  • Analyze the processes and procedures used for scheduling appointments with beneficiaries in order to accommodate the volume and complexity of assistance requests and recommend improvements.
  • Identify gaps in the beneficiary's ability to meet their essential needs and refer complex cases to the Operations Manager.
  • Refer specific cases to the Liaison Nurse of the Red Cross Community Health Directorate.
  • Report challenges encountered during the referral process to the manager.
  • Participate in the department's ongoing development to improve practices.
  • Participate in team meetings.

Reports, administration, and file management:


  • Fill out the necessary paperwork and keep confidential records in accordance with CRC standards using the applicable systems and tools.
  • Ensure that files are processed and clients are followed up with in accordance with department guidelines.

Additional duties:


  • Participate in training, orientations, or exercises, as required.
  • Work with volunteers.
  • Track statistics or other data relevant to file management or reporting.
  • Perform all other related duties, as required.
  • Be available to participate in emergency operations when the department's help is required.

We're looking for:


  • College diploma in social work or a related health care or social and/or human services field.
  • 1 to 3 years of related experience in social work, case management, or community engagement (or equivalent experience).
  • Experience working with older adults.
  • Valid driver's licence.
  • Experience in case management and analyzing client cases (asset).
  • Experience living or working in a culture other than your own (asset).
  • Understanding of the role and mission of the CRC.
