SUMMARY OF DUTIES & RESPONSIBILITIES


Under the general direction of the Family Connections Team Leader, the Family Connections Worker is responsible for completing foster/adoption/kin SAFE home studies and exploring permanency options for children in care, the recruitment and assessment of potential applicants and supporting PRIDE pre-service training.

In addition, the Family Connections Worker is responsible for matching and organizing permanent placements for children and youth in care.

They provide coaching and mentoring and hands on support to the foster/adoptive/kin families and additionally offer post adoption support.


Assess prospective foster/kin/adoptive families (SAFE homestudies), provide clinical support for current family(s), and ensure standards of care in all homes.

Ensures the ongoing development of each foster, adoption and kin family towards providing permanency outcomes for children in care. Match children to permanent and long-term placements.


Carries out all necessary documentation; recordings, court documentations, openness orders, Ministry adoption forms, subsidy agreements, annual reviews, case notes, child data change forms, referrals, social and medical histories etc.

Attend all necessary conferences and meetings. Match child's needs with most suitable placement options and assist all collaterals in maintaining the placement.

PRINCIPAL RESPONSIBILITIES

  • Complete SAFE home studies on foster, adoption and kin applicants
  • Contribute and assist with the agency recruitment strategy. This includes assisting the recruiters in conducting information nights
  • Prescreen applicants to ensure that they meet the agencies requirements for foster and adoption
  • Assist with PRIDE preservice training provided to all foster and adoption applicants
  • Coordinate the collection of appropriate documentation and facilitate the completion of SAFE homestudies
  • Participation in internal committees and Central Region Committees as required
  • Collection of information and writing reports related to social and medical histories of children and birth parents
  • Utilizing Family Finding in making appropriate matching for children in care
  • Complete regular home visits, privacy visits and annual reviews
  • Support families through complaints, allegations and investigations
  • Provide guidance and support through a collaborative approach to teamwork
  • Promote the in care team approach to decision making and ensure inclusivity for all foster and kin parents
  • Attend all internal planning meetings
  • Complete all adoption and/or legal custody documentation
  • Complete all court documents and ministry documents for the purposes of adoption
  • Work collaboratively with all members of the in care team
  • Have knowledge related to targeted and permanency subsidy. Be able to create subsidy contracts and show comprehension in collecting subsidy related statistics and projections
  • Oversee and support children on Adoption Probation and have knowledge of Children in Care Ministry Standards
  • Liaison with Adopt Ontario to resource for children and families
  • Carries out other duties as assigned
  • JOB SPECIFICATIONS/COMPETENCIES
  • Excellent skills in child welfare case management
  • Excellent verbal and written communication skills particularly with assessment reports, goal setting and communication with other professionals and collaterals
  • Demonstrate the ability to coach, mentor and develop caregivers
  • Excellent clinical skills, one on one support and group intervention
  • Contribute to team related data collection
  • Advance skills in mediation involvement multiple parties and between large group
  • Experience in child management, counseling and group work
  • Practice from a Signs of Safety, Family Finding and Anti-Oppressive framework
  • Demonstrated good working relationship with foster parents and families
  • Strong interpersonal skills
  • Good presentation skills
  • Familiarity with Adopt Ontario and Adoption Council of Ontario
  • Demonstrated understanding of diversity, equity, inclusion and the implications on practice
  • Understanding of trends in adoption, openness and maintaining meaningful connections for children and youth

QUALIFICATIONS

  • BSW or MSW degree from a recognized university and at least two years of related experience in a child protection role OR a Bachelor's degree from a recognized university in a related field, and at least two years of related experience in a child protection role.
  • Minimum of 3 years' experience working in child welfare
  • A Class G Drivers License and daily access to a vehicle
