    Job Description:

    As a member of the Clinical Resource Team (CRT) you will have the opportunity to work across programs and across Trillium Health Partners sites. The CRT staff is a vital component of ensuring uninterrupted patient care delivery by providing coverage for planned absences of inpatient Physiotherapists. This position requires an individual who is highly skilled, agile and adaptable with excellent time management and interprofessional skills. Within the CRT Operations and Practice are supported by a Manager, CRT and Allied Health, and the Professional Practice Leader for Physiotherepy. to learn more about the Clinical Resource Team (CRT).

    The Physiotherapist is accountable for the following:

  • Assessment of patients with physical dysfunction of the cardio-pulmonary, musculo-skeletal and neurological systems
  • Planning and implementation of treatment plans to improve movement/function, alleviate pain, and optimize respiratory function
  • Participate in patient education and discharge planning
  • Document patient assessments and progress in the health record
  • Collaboration with the inter-professional healthcare team, patients and their families to achieve set patient goals and augment the treatment plan as needed
  • Providing education to the interprofessional team to support evidence based care
  • Participating in the education of health professions students
  • Qualifications:

  • Candidate must be a graduate of an accredited School of Physiotherapy
  • Registration with the College of Physiotherapists of Ontario for Independent Practice, in good standing
  • . member preferred
  • Experience treating acute care Adult populations (Medical, Surgical, Oncology, Palliative and Geriatrics)
  • Superior interpersonal and communication skills.
  • Demonstrated effectiveness working independently as well as liaising with the interprofessional team
  • Satisfactory performance and attendance required
