CARE COORDINATOR.

JOB DESCRIPTION SUMMARY


The Care Coordinator is responsible for supporting the Director of Client Services in coordinating our delivery of Home Care services.

They will work with our Care Givers, clients and families to allow our clients to remain at home with the highest standard of living possible.

The Care Coordinator also assists in the scheduling and recruitment of Home Care Aides and Care Givers.

This role also provides Home Care services to our clients and substitutes for Caregivers on an as needed basis.


DUTIES & RESPONSIBILITIES

  • Works closely with the Director to ensure that clients are properly scheduled in a customer friendly and efficient way. The Care Coordinator may be required to work scheduled shifts and/or cover for absent Caregivers.
  • Introduces Home Care Aides to clients and familiarizes aides with the Care Plans and adjustments to Care Plans;
  • Liaises with clients and families in the client's spaces as well as from our Home Office;
  • Responsive to client and family requests and concerns and advocates on their behalf
  • Participates in Caregiver selection processes including recruitment, hiring and onboarding of staff;
  • Monitors and facilitates effective and efficient client care documentation through our mobile app;
  • Performs supervisory visits to clients on a regular basis, in compliance with Qualicare policies and procedures;
  • Responsive to caregiver requests and concerns and acts as an advocate on their behalf;
  • Participates in staff development including caregiver orientation, inservice education, continuing education, and evaluation of staff;
  • Verifies credentials and qualifications for Caregivers;
  • Participates and supports the Director of Client Services in administrative and client care policies and practices, while maintaining Qualicare's standards of service;

EDUCATION/EXPERIENCE REQUIREMENTS

  • Minimum two (2) years of clinical experience. One (1) year or more in the Home Health sector.
  • Supervisory experience in a health care setting.
  • Excellent customer service skills and ability to deal tactfully with clients, families, and Caregivers.
  • Understands Alberta and federal regulatory compliance.
  • A valid Class 5 Driver's License and the ability to travel to client's home in their personal vehicle.
  • Current Vulnerably Sector Police Information Check.


The Care Coordinator may be required to be
on call after regular business hours as per the needs of the business.

This position works a hybrid schedule, with the opportunity to work from home on select days.

The Care Coordinator must be willing to travel to client's homes, as frequently as daily, as per the needs of the business.
