Job purpose
The Harm Reduction Worker is a key member of the Health Care Team at Northreach Society in the full continuum of supporting people experiencing homelessness or in need of extra supports as well as STBBI prevention and support.
The Harm Reduction Worker will work in collaboration with members of the nursing team to provide client-centered care to our clients.
Site:
Coordinated Care Campus and Northreach Main Office.
The intent of the Coordinated Care Campus is to provide place-based housing with additional supports to address resident's physical, mental and psychological health, as well as addressing addictions through a harm reduction and recovery focused lens.
Duties and responsibilities
Job Duties Include:
- Provide transportation to clients for various appointments and health services access.
- Promote Opioid Agonist Therapy (OAT) and assist clients to access OAT services.
- Deliver naloxone education and be readily able to respond to opiate poisonings.
- Support local internal/external healthcare providers with STBBI testing events.
- Document all necessary information for reporting requirements.
- Engage in educational opportunities and ongoing professional development relevant to position and organizational mission.
Qualifications
Required Qualifications:
- Experience, education or a combination of experience and education in social services, social work, or health promotion.
- Current CPR/First Aid.
- Valid driver's license and reliable vehicle. Must be able to travel as required.
- Ability to work independently and as part of a team.
- Adaptable and Resourceful
- Strong communication skills with a culturally sensitive approach.

- Maintain confidentiality.
- Organization and accountability
- Accurate and timely data input and tracking
Preferred Qualifications:
- Two years experience working with both housed and street engaged populations providing wrap around services.
- Two years experience working with people who are marginalized.
- Knowledge of HIV/AIDS, Viral Hepatitis, Syphilis, and other STBBI.
- Knowledge of community supports available to clients.

1.0 Full Time
Positions available: 1
Shifts: 10 hour shifts, 4 days on 4 days off. Day shifts. Evening shifts. Weekdays. Weekends. Holidays.
Salary:
Commensurate based on experience.
