Job summary
The Children's Rehabilitation Services program has an exciting opportunity for a Social Worker to serve as a Community Services Coordinator on our team.You will work collaboratively with a multi-disciplinary team providing support and case coordination to address the health needs of clients and families.
In this position, you will receive referrals from community partners (including families, public health, and physicians) and act as liaison with those partners.
