Job summary
We are seeking an experienced registered practical nurse to work as a Community Independence Care Coordinator (Status: Temporary, Full-Time). In this role, you will be responsible for assessment and service planning for low needs patients with a focus on patient independence through system navigation and referral to appropriate community resources.
In collaboration with patients and their families, assess care needs determine eligibility for services develop individual care plans link patients with service providers coordinate monitor care plan delivery establish helping relationships balance patient needs choices available resources ensure patients values preferences respected
Whether you work in our office in the community or a health care facility you will play lead role providing connected accessible patient-centred care supported by collaborative team including over 9000 regulated health professionals
