Options Community Services is a non-profit registered charity providing social services primarily in Surrey, Delta, White

community.

Home Based Addictions Support and Detox program aims to reduce the burden of severe substance abuse, in particular

a reduction in mortality due to overdose events in patient's own homes, increase engagement and retention in

treatment and connection with community resources to facilitate ongoing remission. This program supports clients and

their family to manage their symptoms of withdrawal in the comfort and support of their own home environment in a

safe manner in consultation with family physicians and other clinical support. This goal of the program is to ensure

greater accessibility and awareness through culturally competent and language specific service delivery for the

individual and their family.

The Outreach worker for Home Based Addiction Support and Detox program will be responsible for providing culturally

agile services that focus on addressing addiction issues, promoting overall health and emotional well-being, and

connecting clients to additional resources such as counsellors and nurses. This position will work closely with clients

and their family members to develop and implement personalized support plans, and will be responsible for

maintaining accurate records and reporting progress to Program Managers.


ACCOUNTABILITIES

  • Teach clients, families and/or significant others a variety of coping mechanisms and strategies focusing on a
healthy lifestyle including self-care, breathing techniques, and healthy eating.

  • Assist in evaluating the effectiveness of treatment programs by tracking clients' behavioural changes and
responses to interventions.

  • Meet with clients to assess their progress, give emotional support and discuss any difficulties or problems.
  • Participates as a team member with other staff to ensure a safe and caring environment by performing duties
such as responding to emergency issues, attending general and team meetings and supporting others through
methods such as sharing of knowledge and information.

  • Participate in agency and program specific quality improvement initiatives.
  • Participate in regular clinical and administrative supervision according to accreditation standards.
  • Assesses client needs and connects the client and family with an addiction counsellor as well as Nurse.
  • Accompany client for appointment in the community, as necessary.
  • Creates a recovery plan based on initial assessment and refers clients to available community resources to
provide additional help where needed.

  • Develops plans of action and followsup of the client goal plan.
  • Assist clients in locating and utilizing community resources including medical, housing, employment, daycare,
and other services.

  • Support the client during all stages of recovery from usage to abstinence.

  • Develop discharge plans in partnership with the treatment team.
  • Complete client documentation and data requirements in accordance with program
  • Maintain client and service records and prepare reports in accordance with COA standards and agency policy.
  • Maintain contact with other social service agencies and health care providers involved with clients to provide
information and obtain feedback on clients' overall progress

  • Perform other related duties as required or directed.

QUALIFICATIONS

Education, Training, and Experience:


  • Bachelor's degree in social services or a related field and a minimum of 2 years experience working with clients
with addiction issues or the vulnerable population.

  • Fluency in Punjabi is required.
  • Valid driver's license and access to an appropriately insured personal vehicle for work purposes.
  • Experience in home visitation is an asset.

Skills and Abilities:


  • Current Level I First Aid certificate.
  • Demonstrated effectiveness as part of a staff team.
  • Awareness and understanding of the key roles that a person's culture, identity, ethnicity, race, gender and
other aspects of diversity play in the counselling process

  • Knowledge of addictionsbased practices
  • Demonstrated proficiency in written, oral, interpersonal communication, and report writing skills.
  • Demonstrated ability to develop program related reference and resource materials.
