Description

Care Manager ensures quality of client care by supervising Community Health Workers (CHW) in Vancouver and Victoria, performing routine community visits, working collaboratively with agency staff and with other care professionals involved in the provision of care.

Perform duties within the competency guidelines and standards of practice outlined by the college of LPNs/RNs of BC and the current legislative scope of practice in a community health setting.

Key Duties and Responsibilities:
  • Provides guidance to service co
  • ordination to ensure effective day to day operations
  • Supervises CHW's by providing ongoing feedback and conducting probationary and annual performance reviews.
  • Participates in orientation, training and setting of performance expectations of new CHWs
  • Maintains and fosters effective and timely communication with Schedulers and CHWs
  • Recommends training requirements for individual CHW's and assists with ongoing CHW educational opportunities, including providing inhouse education
  • Identifies skills and/or knowledge gaps relative to the CHW program operations and ensure the staff have the skill set to meet individual client needs
  • Provides consultation and training for delegable tasks. Must have an in depth understanding of the Personal Assistance Guidelines (2008)
  • Conducts client visits and updates care plan information. Oversees the implementation of individual care plans as determined by assessment. Communicates client care plans to CHW's and Schedulers
  • Documents, investigates, and responds to inquiries, client feedback and concerns related to the delivery of home support services by investigating CHW practice issues and taking required and timely action
  • Provides information to CHW's, clients, and families with regard to available resources
  • Provides advocacy services on behalf of CHW's clients, and families
  • Assists with Worksafe BC accident investigation followup as required including home visits to review and ascertain events that led to the accident. Reports information verbally and in writing to Chair, OH & S committee
  • Maintains pertinent records (i.e., employee files paper and dated notes, visit reports) including computer records as required
  • Practices Performance Management and Participates in CHW corrective practice discussions as required
  • Provides back up for other teams during any absences
  • Rotates on call as necessary
  • Collaborates with Schedulers to effectively deploy CHWs recognizing the impact of key cost drivers i.e. travel time, mileage, etc.
  • Follows employee relations processes and relevant policies and procedures
  • Maintains up to date knowledge of relevant agency policies and procedures and recommends changes as appropriate
  • Completes data collection for chart audit and other quality reports
  • All other reasonable duties as assigned

Qualifications:


Education, Training and Experience:


  • Full scope RPN Certification, current BCCNP registration required.
  • Full scope of RN Certification, current BCCNP registration required.
  • Equivalent education, training, and experience as determined by the employer.
  • Training and proficiency in computer operations.
  • Oneyear previous nursing experience in a health care setting.
  • Oneyear of supervisor experience

  • Communicate effectively both verbally and in writing
  • Supervisory experience and skills
  • Independent worker with a minimum of supervision
  • Good health, physically and mentally able to carry out the assigned duties in the working environment
  • Ability to establish and maintain harmonious working relationships, and to participate effectively as a team member
  • Ability to recognize changes in clients
  • Computer skills
  • Must have a valid driver's license and access to a personal vehicle
