Description

Under the general supervision of the Director of Care, the Social Services Assistant (SSA) coordinates the admission and discharge of residents and provides on-going social services.


Duties

  • Liaises with Vancouver Coastal Health and the Director of Care, to coordinate the admission and discharge of residents; providing orientation to the Lodge and its services, arranging for placement in alternative facilities/programs, arranging in-home assistance, advising families and community agencies of the discharge and making travel and other arrangements. Provides required admission, discharge and transfer documentation to VCH.
  • Interviews residents having financial and/or social difficulties, obtains additional information as required from a variety of sources such as family and nursing staff, refers residents and/or family to a Social Worker and/or other appropriate community resources as required.
  • Coordinates weekly resident Care Conferences, liaising with residents, family, BPL staff, pharmacists and physicians as necessary.
  • Provides practical services to residents such as transportation, escort or appointment arrangement, completing registration forms for community activities and obtaining the necessary consent; supports and assists residents to identify additional needs.
  • Cooperates with the Finance team to coordinate resident's financial arrangements, including bank deposits, comfort allowances, collecting co-insurance charges and updating medical plans.
  • Responds to general inquiries, refers callers to internal and external resources such as health teams, physicians, community and government agencies.
  • Arranges and hosts weekly Lodge introductory tours.
  • Performs other related duties as assigned.

Education, Training and Experience
Graduation from a recognized Social Services Worker certificate program. A minimum of one-year related experience.


Skills and Abilities
(i) Ability to communicate effectively both verbally and in writing.

(ii) Proven experience of ability to address and calm heightened emotions in others advantageous.

(iii) Ability to deal with others effectively, including proven experience of dealing with elders struggling with dementia and other mental health issues.

(iv) Physical ability to carry out the duties of the position.

(v) Good organizational skills

(vi) Computer literate with previous experience of pointclickcare an advantage.
