TEAM ASSISTANT, HOSPICE PALLIATIVE CARE

Permanent, Part-Time (0.5 FTE)

KEY RESPONSIBILITIES

  • Providing administrative support for KW, Guelph, Cambridge HPC team: HPC Physicians, Nurse Practitioner and Clinical Nurse
  • Providing reception in Physician office
  • Creating medical charts for Physicians and Nurse Practitioners
  • Managing high volume of incoming faxes and phone calls
  • Collecting medical history from various databases (Clinical Connect), physicians, and service providers to create and maintain patient information
  • Daily uploading of lab reports
  • Processing medical orders and prescriptions
  • Liaise with Hospice to send information for patient admissions
  • Creating weekly caseload reports
  • Authorizing and completing Service Offers
  • Updating and maintaining current client information and records in CHRIS database, including client's service bookings
  • Completing physician requests to obtain patient test results
  • Notifying patient of test appointments
  • Helping patients with Insurance forms
  • Support Homeless and Vulnerably housed patients
- tracking down health history

  • Uploading NP notes to Meditech
  • Assembling and maintaining files of pertinent client information; verifying that all relevant documents have been completed and filed
  • Booking home visits and client conferences as needed
  • Generating reports and labels from CHRIS
  • Securing and/or shredding confidential documents as required
  • Transcribing, copying and distributing letters, memos and conference reports using word processing software and other equipment
  • Performing other duties as assigned.

EDUCATION AND EXPERIENCE

  • Successful completion of grade 12, plus an additional program of up to one year in duration;
  • Two to three years of relevant administrative office experience;
  • Knowledge or experience in medical terminology an asset;
  • Understanding of available community resources, services and the roles of health care professionals, hospital and support workers;
  • Proficient in the operation of a personal computer in a networked environment using computer software and data entry skills;
  • Able to communicate effectively in both oral and written formats with internal staff and external clients;
  • Knowledge of office systems, procedures and business practices;
  • Able to work effectively in a team based setting, strong planning and organizational skills, along with the ability to multitask and respond to changing priorities, and to work independently;
  • Able to relate effectively with sensitivity, tact and discretion to clients, family members, staff and the public, along with ability to maintain confidentiality.
  • Fluency in French language preferred.


This position may be required to provide relief at other Home and Community Care Support Services locations or teams as needed and may be part of the on call rotation as per the provisions of the ONA collective agreement.
