About the Role
We are seeking a skilled Healthcare Continuity Specialist to join our team at Nova Scotia Health. As a key member of our healthcare team, you will be responsible for assessing clients' needs, developing personalized care plans, and authorizing services to ensure seamless continuity of care.
The ideal candidate will have a Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies, as well as current registration with the applicable Nova Scotia Professional Regulatory body. A minimum of three years' experience in a community or long-term care setting is required, along with demonstrated knowledge of community agencies and resources.
You will work closely with clients, family members, caregivers, and healthcare teams to facilitate discharge planning, provide ongoing case management, and explore alternative solutions when necessary. Your excellent assessment, interviewing, and negotiation skills will enable you to build strong relationships and navigate complex situations effectively.
As a Healthcare Continuity Specialist, you will be fiscally responsible for authorized services within available departmental policies and programs. You will maintain ongoing communication with stakeholders, ensuring that clients receive comprehensive support and services.
