    Department/Unit Summary:

    4J Neurosurgery and 3J Neurosurgery Special Care Unit provide care for patients admitted electively for planned surgery as well as through our Emergency Department and Critical Care services.

    4J Neurosurgery is a 14-bed unit that supports the care of patients who have been diagnosed with brain tumours, subdural hematoma, subarachnoid hemorrhage and arterial venous malformations. As the Regional Centre for Neurosurgery, 4J Neurosurgery offers a wealth of learning opportunities in peri-operative nursing.

    3J Neurosurgery Special Care Unit is an 8-bed unit with a nursing ratio 2:1 that provides high observation to Neurosurgery patients post-operatively and patients with higher care needs than can be managed on a ward level.

    Together, 4J Neurosurgery and 3J Special Care Unit are supported by an inter-disciplinary team of RNs and RPNs, CQCL, Charge Nurses, Physician Assistants, Physiotherapists, Occupational Therapists, OTA/PTA, Speech Language Pathologists, Discharge Planners, Dieticians, Pharmacists and Neurosurgeons. The team provides exceptional, patient-centred care in a collaborative and integrated manner. As a part of an academic centre, our team participates in ongoing research, quality improvement and professional development initiatives.

    Position Summary:

    The Clinical Out Reach Specialist (CORS) is a Healthcare Practitioner with a graduate education in healthcare, possessing advanced clinical and theoretical knowledge and skill in neurosurgery/spine nursing. This role encompasses multiple dimensions that fall within the domains of advanced nursing practice including: Clinical Practice, Research, Leadership, Consultation and Collaboration. The (CORS) will develop effective partnerships with physicians and other members of the interdisciplinary team within THP as well as at regional partner organizations in Mississauga/Halton and Central West sites to support quality of care to patients and their families. Additionally, the CORS will play a key leadership role in evaluating quality of care and patient outcomes within the specialty population, and will build inter-organizational capacity to improve the health care experience for people living with life limiting illness, and their family.

    Key Responsibilities:

  • Establish a comprehensive neurosurgical transition repatriation process for the neurosurgical program; this will include strategies to attain patient and family feedback and develop models for successful transition to the community
  • Initiate patient consultations within the acute phase, following patients and families until successful transition to the discharge destination
  • Collaborate with the various team members, including both internal and external stakeholders to enhance patient transition
  • Create and implement a linkage process to ensure relationship development at each of the non-neurosurgical community hospitals in LHINS 5 and 6, CCAC, rehabilitation facilities
  • Collaborate with the Regional Neurosurgery APN to develop and facilitate resource exchange to enhance learning and address knowledge gaps in the community
  • Collaborate as a member of the team for internal neurosurgical initiatives
  • Develop and facilitate a neurosurgical outreach working group in order to plan both processes and resources required for patient transition
  • Facilitate mentorship/coaching opportunities for members of the team
  • Integrate evidence-informed literature within all components of the role
  • Determine opportunities to develop research projects that address transition and outreach challenges for neurosurgical patients
  • Report to the Director of Neuro/MSK and collaborate with members of the team
  • This position will require travel to hospitals in Mississauga Halton and Central West Regions
  • Key Qualifications:

  • Baccalaureate degree in a Health discipline required (Nursing or Allied Health)
  • Current registration and membership with appropriate College or Association
  • Completed Master's degree in Nursing or healthcare discipline
  • 5-8 years' experience with demonstrated excellence in acute neurosurgical care
  • Effective leadership skills to facilitate project development and change
  • Well-developed communication skills and ability to establish positive relationships with staff, patients, families, inter-professional team and community agencies
  • Superior interpersonal and coaching skills to facilitate development and implementation of a neurosurgical outreach model to enhance patient transition from acute care
  • Demonstrated program planning, implementation and evaluation skills
  • Experience with identifying, disseminating and implementing evidence-informed practice policies and care practices for the neurosurgical population an asset
